| Purpose:To understand the expression of CD15 in AML,to investigate the clinical features of CD15~+AML patients and to analyse the value of CD15 in evaluation of AML prognosis.Method:127 adult AML patients,who were initially treated in our hospital from January 2010 to October 2017,was collected.Flow cytometry(FCM)was used to test expression of cell differentiation antigen.The patients were then divided into the CD15~+group and the CD15~-group according to expression of CD15,and compared for their clinical features,2-course complete remission(CR)rate and overall survival(OS).Making Univariate and multivariate analysis of OS of the AML patients.Result:1.In 127 AML patients,CD15 expression was found in 77 cases,the positive rate was 60.63%.CD15 was expressed in AML subtypes(M1~M6),highest in M5(83.3%),followed by M4(70.0%).2.The proportions of male patients and patients aged below 60 years old in the CD15~+group are both significantly higher than those in the CD15~-group(P<0.05).3.The WBC count,percentage of peripheral blood blast cell and percentage of bone marrow blast cell at first visit are significantly lower in the CD15~+group than in the CD15~-group(P<0.05),while the two groups show no statistical difference of hemoglobin and platelet(P>0.05).4.Comparing with the CD15~-group,the proportion of patients with low cytogenetical risk is slightly higher in the CD15~+group(P>0.05),while the proportions of ones with moderate and high cytogenetical risks are both decreased in the CD15~+group.The differences show no statistical significance.There was no statistical difference in t(8;21)between the two groups(P>0.05).5.The 2-course CR rate for the CD15~+group is significantly higher than the CD15~-group(P<0.05).Comparing with the CD15~-group,the CD15~+group has a significantly longer OS(13 mo.vs.8 mo.for the CD15~+group and the CD15~-group respectively)(P<0.05).6.Univariate analysis showed that it were age,newly diagnosed WBC count,proportion of bone marrow blasts,karyotype,induction therapy efficacy,and CD15were related factors affecting OS in AML patients.In the multivariate analysis,the newly diagnosed WBC count was an independent factor affecting the prognosis of AML patients.Conclusion:1.The positive expression rate of CD15 in AML patients was 60.63%,and the highest expression in M5.2.CD15~+AML is mostly seen in male patients,and is characterized by lower WBC count,rate of peripheral blood blast cell and rate of bone marrow blast cell at first visit.3.CD15~+AML patients have a higher complete remission rate and longer overall survival.4.The newly diagnosed WBC count is an independent prognostic factor for AML patients,CD15 is not an independent prognostic factor for AML patients.5.The prognostic significance of CD15 antigen is limited,and its significance for the prognosis of AML remains to be further explored. |
