| Objective To explore the relationship between CT curve and single-energy CT value and the degree of differentiation of renal clear cell carcinoma in dual-source CT,so as to predict the degree of differentiation of renal clear cell carcinoma with imaging and not pathological results,and provides an early reference for the appropriate surgical procedure.Method Fifty-eight patients with renal clear cell carcinoma confirmed by pathology were collected,Preoperative dual-source CT skin marrow and real-time dual-energy mode scan were implemented.In which high differentiation group(Ⅰ grade,Ⅱ grade)in 32 cases,low differentiation group(grade Ⅲ,grade Ⅳ)in 26 cases.Siemens second generation Hyun speed double source CT was adopted.The scanned image data is transmitted to the workstation,the two sets of thin-layer data were transferred to Dual Energy software,select the "Monoenergetic" application to obtain the spectral attenuation curve of renal clear cell carcinoma lesions at 40-140 kev,measure the corresponding CT value of renal clear cell carcinoma lesions at 40-140 kev of skin marrow and the actual period.And spectral attenuation curve is divided into 40-70 kev,70-100 kev,100-140 kev three sections,calculate the slope of 3 sections respectively,the specific formula is K40-70kev=(40kev-70kev)HU/30,K70-100kev=(70kev-100kev)HU/30 K100-140kev=(100kev-140kev)HU/40.One-way analysis of variance(ANOVA)was used to compare the difference between the slope of the energy spectrum decay curve and the CT value under single energy in highly differentiated renal clear cell carcinoma and poorly differentiated renal clear cell carcinoma.Result A total of 58 patients,Pathological results were well-differentiated group of 32 cases(Ⅰ,Ⅱ grade);poorly differentiated group of 26 cases(Ⅲ,Ⅳ grade).Highly differentiated group of renal clear cell carcinoma and poorly differentiated group of renal clear cell carcinoma between the skin stage 40-70 kev,parenchymal 40-100 kev level of single energy CT value difference was statistically significant,P<0.05;The skin of 80-140 kev and substantive stage 110-140 kev level of single energy CT value difference was not statistically significant(P>0.05).The skin stage 40-70 kev,70-100 kev and parenchymal 40-70 kev,70-100 kev segment slope of high differentiated group of renal clear cell carcinoma of were greater than poorly differentiated renal clear cell carcinoma,the difference was statistically significant(P<0.05);There was no significant difference between the two groups in the period of 100-140 kev in the periodontal and 100-140 kev in the pus(P>0.05).Conclusion The spectral attenuation curve and the CT value under the single energy of CT can help differentiate the differentiation of renal clear cell carcinoma,Contribute to the selection of clinical renal clear cell carcinoma treatment options and prognosis of renal clear cell carcinoma. |
