The Hamilton Clinical Anxiety Scale Was Used To Evaluate The Clinical Efficacy Of Dialectical Treatment Of Panic Disorder

IntroductionPanic Disorder (PD) is a common seen mental and spiritual disease in modern clinical treatment which is difficult to cure and seriously affect the quality of patients’life and their social functions. With the modern social work, interpersonal relationships, factors such as the role of economic pressure intensifies, the incidence of panic disorder increases year by year, which has brought negative impact to the family and society. The rate of suicide thoughts or suicide attempts in people with PD is obviously higher than other spiritual disorder patients. Neither five selective serotonin reuptake inhibitors nor cognitive behavioral therapy has insufficient such as effecting slowly, taking a long time or high recurrence.The word "panic disorder" is not included in Chinese medicine. From the clinical perspective, the disease is an emotional illness, which belongs to heart disease.In recent years, some Chinese experts have started to research on panic disorder, and have achieved certain results. The Chinese medicine in the research, which has obvious effect in clinical application, is optimally formed by director Yang Guohua on the basis of long-term clinical experience. This subject carries out the randomized controll study, in order to objectively evaluate its clinical efficacy.ObjectiveUse the Hamilton Anxiety Scale to observe the clinical efficacy and safety of treating panic disorder by Chinese decoction based on syndrome differentiation.MethodsThis subject adopts a prospective multicenter and central randomized control study. The cases are selected from out-patient in Wangjing Hospital of China Academy of Chinese Medical Sciences, Beijing Chaoyang Hospital and Beijing Dongfang Hospital during December2010to November2011. The cases in Chinese medicine treatment group use medicine based on syndrome differentiation, at every time200ml, twice daily; The cases in western medicine control group use seroxat, at every time20mg, take it every morning. The course of treatment was4weeks.Observe the score of HAMA, at curing front, curing1day,3days,5days,7days,14days,21days,28days,after curing1month,2months and3months. Determine the safety index which involves routine blood, urine, electrocardiograms and liver function before treatment and four weeks after treatment, at the same time record treatment emergent symptom scale. We analyze the data, the count materials are examined with x2, the materials are examined with t-text and the grade materials with wilcoxon. All data are used SPSS13.0analyzed statistically.Results1The score of HAM A in every groupThe symptoms of Panic Disorder could be improved by both the Chinese medicine treatment group and the western medicine control group. There were no differences between the two groups on the score of HAMA before treatment (P>0.05). Score was significantly reduced after treatment. By the paired t-test, there was significant differences before and after treatment in every group (P<0.01).2The comparison of clinical efficacy between the two groupsThe Chinese medicine treatment group showed higher total effective rate than the western medicine control group. The total effective rate was100%in the Chinese medicine treatment group, and93%in the western medicine control group. After four weeks treatment, the HAMA scores and difference of the two groups have significant statistical difference (P<0.01).This shows that the total effective rate of the Chinese medicine treatment group is higher than the western medicine control group.3The comparison of long-term efficacy between the two groupsThe long-term efficacy of the Chinese medicine treatment group was better than the western medicine control group. Compare the score of after-treatment and3months follow-up, differences of the score difference between the two groups showed statistical significance.4Drug safety analysis between the two groupsNo serious adverse reaction was found in the two groups. The adverse drug reactions are mainly for dry mouth, sweating, lethargy, diarrhea and so on. Chinese traditional medicine group’s adverse reaction is less than western medicine group.ConclusionThe clinical curative effect of the Chinese medicine treatment group and western medicine control group are both satisfied. The comparison of HAMA scores proofs that the short and long term curative effect of the Chinese medicine treatment group is better than the western medicine control group, adverse reaction is less than western medicine and Panic disorder treatment of using "Chinese decoction which can nourish the heart and gallbladder and soothe nerves based on syndrome differentiation" is safe and effective.
