Clinical Study On The Treatment Of Moderate To Severe Acute Pancreatitis With Jiawei Dachengqi Decoction

Objective: Through clinical observation Modified Dachengqi Decoction treating mild acute pancreatitis(MSAP) and the clinical effect of the treatment of severe acute pancreatitis(SAP),explore the discharge heat dispelling stasis as the main therapeutic principle of Modified Dachengqi Decoction to improve the MSAP and SAP related mechanism of compound for traditional Chinese medicine treatment of MSAP and SAP provides a safe and effective method.Method: 60 patients with SAP MSAP and completely randomly divided into control group and treatment group, 30 in each group, control group to conventional western medicine treatment, the treatment group on the basis of conventional western medicine combined with Modified Dachengqi Decoction flavored by stomach tube slow injection, clip stomach tube1.5 h, 3 times a day, each 150 ml medicinal broth, observation of 1, 4, and 7 days after the hospital of D-Dimer, blood amylase, PT, c-reactive protein, intra-abdominal pressure(IAP),CTST score, abdominal signs and symptoms(abdominal pain, abdominal distension, the bowel sounds, nausea, and vomiting integral) the change of value, compare the curative effect of two groups of patients.Results:1.The control group and treatment group there was no statistically significant difference of each index before treatment(P > 0.05). Treatment group and control group in the treatment of1, 4, and 7 days after the serum amylase, C- reactive protein, PT, D-Dimer, CT grading integral value decreased, and the treatment group than the control group decreased more significantly(P < 0.05).2.Treatment group and control group 1, 4, and 7 days after treatment in patients with abdominal pain, abdominal distension, bowel sounds, nausea, and vomiting all integral and IAP value decreased, the treatment group than the control group decreased more wide(P <0.05). Differences between the two groups have statistical significance, Modified Dachengqi Decoction + western medicine treatment compared with the pure western medicine treatment has more advantages in the treatment of moderately severe acute pancreatitis.Conclusions:1.Modified Dachengqi Decoction by promoting blood circulation and removing blood stasis can improve microcirculation in patients with MSAP and SAP, thus reduce moderately severe acute pancreatitis;2.Modified Dachengqi Decoction by purgating fu-organs to eliminate heat can promote MSAP and SAP patients with gastrointestinal peristalsis, reduce intra-abdominal pressure,prevent bacterial translocation, alleviate moderately severe acute pancreatitis.
