Traditional Chinese medicine believes that the basic pathogenesis of rheumatoid arthritis is as the deficiency of qi and blood, Exogenous wind cold dampness as the diagnostic criteria. Caulis Sinomenii combined with Radix Paeoniae Alba has the functions of dispelling wind, cold and dampness, nourishing blood and liver, damping the pain. According to the basic pathogenesis of rheumatoid arthritis, the group is designed to establish a relevant animal model, and to explore sinomenine and paeoniflorin synergistic treatment of RA and RA wind cold dampness syndrome curative effect and mechanism. In order to provide a reliable experimental basis for the clinical application, and provide ideas for the research and development of new drugs for the treatment of RA.Objective: To investigate the effect and related mechanism of sinomenine combined with paeoniflorin in teatment of RA and RA wind cold dampness syndrome.Methods:(1) Used the experiment of mouse ear swelling which induced by xylene and Kim Q value method, screening and evaluation of sinomenine compatibility of paeoniflorin optimal dose.(2) Through the CIA model to observe the influences of the body weight,inflammation score, histopathology, radiology about rat by sinomenine combined with paeoniflorin.(3) Through the method of Enzyme-Linked Immuno-Sorbent Assay technology to explore the prion-inflammatory factor of TNF-α、IL-1β expression by SIN and PAE.And using Western blot technology, researched SIN combined with PAE on the expression about the protein of MMP-2、MMP-9、TIMP-1 reaction in bone.(4) Stimulated the rats by wind cold dampness method and tail injection of collagen method to establish the rat model of RA wind cold dampness syndrome. It’s like human RA wind cold dampness syndrome. Through the model to detect the effect of sinomenine and paeoniflorin synergistic treatment.Results:(1) High and middle dose of sinomenine, high dose of paeoniflorin and high and middle dose of the combined drug group could obviously inhibit the inflammatory swelling of mice auricle, which induced by xylene(P <0.05). The combination group from synergistic effect to additive effect, with increasing doses of sinomenine and paeoniflorin. The best group is middle dose of the combined drug group.(2) Sinomenine, paeoniflorin, combination group could improve the body weight of CIA rats(P <0.05、P <0.001). Effective inhibited rat paw swelling(P <0.05、P<0.001), Inflammation score(P <0.05、P <0.001), histopathological score(P <0.05,P <0.001), Radiographic score(P <0.01,P <0.001). Moreover, the combined group was more effective than the single drug group.(3) Sinomenine, paeoniflorin, combination group could obviously inhibit the expression of TNF-α, IL-1β(P <0.001) in serum and the expression of MMP-2 、MMP-9(P <0.001) in bone tissue of CIA rats. The combined group was more effective than the single drug group(P <0.001).(4) Sinomenine, paeoniflorin, combination group could improve the body weight(P <0.01), inhibited rat paw swelling(P <0.05、P <0.01) and Inflammation score(P<0.01) in CIA cold wet dampness composite rat, reduced inflammatory cell infiltration, proliferation of synovial, erosion of cartilage.Conclusion:(1) Sinomenine, paeoniflorin and combination group were significantly improved on collagen induced arthritis in rats. Sinomenine and paeoniflorin collaborative mechanism in treating CIA may be blocked the chain of inflammatory reaction through prion-inflammatory factor of TNF-α, IL-1β. It also may be raised the expression of TIMP-1, inhibited the expression of MMP-2 and MMP-9.(2) Through CIA cold wet dampness composite model similar with the arthralgia due to wind cold dampness to found sinomenine-paeoniflorin combination therapy for the treatment of RA with wind cold dampness syndrome has good potential.
