| Purpose: To analyse the risking factors of cognitive impairment in maintenance hemodialysis patients and the type characteristics of TCM Syndrome.Material and method: 216 Cases were selected from Shenyang military region general hospital blood purification center who were on maintenance hemodialysis and without intracraniai organic diseases by head CT and Mlu examinations.The cognitive impairment in maintenance hemodialysis(MHD) patients were evaluated by the Montreal Cognitive Assessment(Mo CA),and the clinical parameters were compared between the non-cognitive impairment group and cognitive impairment group.There were 117 cases in non-cognitive impairment group(MOCA>26 scores),and 99 cases in cognitive impairment group(MOCA≤26 scores),we compared the observation indexed and the TCM Syndrome of two groups and analyzed the functional status with cognitive influence factors.Results: The incidence of cognitive impairment increases progressively with age and lower education level,and the difference is` more significant in women than in men.And the incidence of cognitive impairment is higher in patients with primary hypertension.Compared with noncognitive impairment group,the percentage of older patients in cognitive impairment group were significantly higher(55.39±12.56 more than 47.37±11.60),the education level(9.84 ±2.93 less than12.59± 3.16),blood albumin level(40.06±3.15 less than 41.13±4.17) were lower. There were statistically difference in all the observed indicators above between two groups(P<0.05),however, the advanced age and low educational level have statistical significance in multiple regression analysis of cognitive impairment.In the traditional Chinese medicine,the cognitive impairment group is mainly based on deficiency of both Yin and yang, the non-cognitive impairment group with the spleen and kidney qi and yin deficiency, however,in terms of Chinese medicine and syndrome, the two groups are mainly damp heat syndrome.Two in the group of yang deficiency of spleen and kidney,the deficiency syndrome of both Yin and yang, the spleen and kidney qi and yin deficiency are statistically significant(P<0.05).In traditional Chinese medicine,the age is greater than or equal to sixty of thepatients are mainly based on deficiency of both Yin and yang,and the patients who ages are less than sixty years are mainly based on he spleen and kidney qi and yideficiency,however, in terms of Chinese medicine and syndrome, the two groups are mainly damp heat syndrome. The two yin and yang deficiency syndrome,spleen and kidney qi and yin deficiency were statistically significant(P<0.05).Conclusion: The incidence of cognitive impairment in hemodialysis patients is high.And advanced age,low educational level and low albumin level are the vital risk factors,particularly advanced age and low educational level are the independent risk factors for cognitive impairment. Cognitive impairment is more likely to occur in patients with yin and yang deficiency syndrome in two, however, the yang deficiency of spleen and kidney, spleen and kidney yin deficiency syndrome with two less cognitive impairment. Elderly patients in traditional Chinese medicine syndrome is more likely to occur yin and yang deficiency syndrome two, however, younger patients are more likely to suffer from deficiency of both qi and yin of spleen and kidney.Age is an important factor to affect cognitive function and TCM Syndrome Types. |
