Objective: The clinical significance of the small continuous curvilinear capsulorhexis,i.e.if it can improve the success rate of the phacoemulsification of intumescent white cataract.Methods: 18 cases(18 eyes)of intumescent white cataract from February2014 to August 2015 are collected,and 25 cases(25 eyes)from January 2013 to January 2014 are collected for retrospective study.The procedure of small continuous curvilinear capsulorhexis surgery: stained or unstained,break the anterior lens capsule with a discission needle,suck out part of the emulsion and cortex,make a 2mm tinny contrnuous circular capsulorhexis,suck the cortex or chylous fluid with a tip through the capsulorhexis opening,leaving only the lens nucleus in the capsule,the diameter of the secondary capsulorhexis is 5mm to 5.5mm,then conduct the conventional phacoemulsification.Results: As for the 18 eyes in the small continuous curvilinear capsulorhexis group,only 2 cases of anterior capsule rupture are found and there’s no posterior capsular rupture in any cases.CCC success rate is 88.89%.One day after the surgery,there are descemet membrane fold in 3 cases,along with light corneal edema.In contrast,for the control group,we find anterior capsule rupture in 10 cases and CCC success rate is 66.67%.We find posterior capsular rupture in 2 cases,nucleus drop in 1 case,corneal edema in 5 cases,and intraocular pressure increase in 2 cases,which suggest that the success rate of small continuous curvilinear capsulorhexis is higher than that of the conventional capsulorhexis.There ’ re significant difference between the two groups.Conclusions: the small continuous curvilinear capsulorhexis can significantly improve the success rate of the phacoemulsification of intumescent white cataract,making the surgery easier and effectivelyimproving the success rate of phacoemulsification.Objective: To evaluate the therapeutic effect and operative technique of the surgical skills of phacoemusification for hypermature white cataract.Methods: From February of 2013 to August of 2015,24 patients of hypermature white cataract were treated by phacoemusification,which were retrospectively compared with 24 patients treated by regular phacoemusification+ECCE from January of 2013 to May of 2014.Every patient’s naked vision and the best correct vision were examined preoperatively and postoperatively.We have developed a new way to finish the hypermature white cataract,we call it “Pre 一 implant IOL method” : First,the core is divided into two parts,and the 1/2 core is divided into 2 pieces,and then the pulse energy is emulsified.In the pocket of the injection of viscoelastic agent,at this time in the lens implantation under the lens in the pocket,under the protection of intraocular lens,the remaining 1/2 core block and then divided into 2 blocks,conventional ultra milk removal of the remaining 1/2 core,suction and residual cortex,complete the operation.Results: In experiment group,23 cases achieved thorough capsular integrity,unilateral anterior capsular tear in 1 case,theree were no posterior capsular rupture;In control group,17 cases were achieved thorough capsular integrity,unilateral anterior capsular tear in 2 case,bilateral anterior capsular tear in 1 case,posterior capsular rupture in 4 case;there was signification difference between the two methoda for the safety of capsular integrity(p <0.05).The corneal edema in experiment group was less at 1d and 1w afertsurgery(p<0.05).In experiment group the endothelial cells on 1 month are2314±278/m compared with In control group.The rate of loss corneal endothelial count between two groups had no signification difference(P>0.05).Conclusions: With ‘Pre 一 implant IOL method’Phaco,the hypermature white cataract could betreated safely.
