| Objective:To study the anorectal motility and sensory characteristics of patients with different TCM syndromes of chronic constipation,to make TCM syndrome types objective,to improve the accuracy of TCM syndrome differentiation,to guide clinical medication better,and to obtain better therapeutic effect.Methods:Anorectal manometry was used to measure the anorectal motility and sensory function of constipation patients with chronic constipation by professional and technical personnel.Analysis of TCM Syndrome Distribution in patients with chronic constipation by using Statistical Software.Study on the correlation between the specific syndromes of chronic constipation and the data of anorectal manometry.At the same time,the basic information of the patients that will be gathered,Such as age,sex,eating habits,BMI,family economic situation and education level,whether there is a positive family history of constipation,etc.Understand whether these factors are associated with chronic constipation.Results:1.Chronic constipation patients are mainly female(68%),Female:male= 1.56,The average age was 52.21 ± 15.61 years old,and 85 cases were mainly middle and old patients aged 40 to 75 years.2.Chronic constipation as a common,multiple chronic gastrointestinal disease,because of the repeated attacks of constipation symptoms and the persistence of constipation symptoms,cause patients to repeatedly seek medical treatment,most of the patients in this study(79%)have repeated medical treatment.The experience of repeated drug therapy,and the main choice of drug treatment were Chinese patent medicine(65%)and western medicine(62%).3.There is a correlation between the occurrence of chronic constipation and body mass index(BMI).In this study,the number of patients with normal body mass index(BMI)is 74,accounting for 74%.Compared to obese patients,overweight patients and those with low body weight are the most common ones.4.Some unhealthy lifestyle and eating habits were associated with the occurrence of chronic constipation and were risk factors of chronic constipation.In this study 84 patients had at least 2 items of bad life and eating habits.5.Chronic constipation was classified according to TCM syndrome differentiation,and it was found that chronic constipation was more common than deficiency syndrome(P<0.05).In the specific syndrome differentiation,Qi stagnation,heat accumulation and qi deficiency were the common syndromes of chronic constipation.Qi stagnation was the most common syndrome type of chronic constipation(P<0.05),age was related to deficiency syndrome type of chronic constipation,and the age of deficiency syndrome group was larger than that of positive group(P<0.05).6.In this study,the anal resting pressure and anal systolic pressure in the experimental group were higher than those in the deficiency syndrome group(P<0.01),and the anal resting pressure and anal systolic pressure in the deficiency syndrome group were higher than those in the yang deficiency constipation group(P<0.05).Defecating in most of the patients had anal sphincter relaxation or contraction of bad contradiction(82%),normal rectal pressure can not be increased(64%),of which 37 cases of patients with anal defecating in the residual pressure does not decrease but increased,45 patients in the simulated defecation anus when the residual pressure can be decreased but did not decrease enough.The anal residual pressure in patients with chronic constipation was higher than that in patients with deficiency syndrome(P<0.05),and the residual pressure in patients with Qi deficiency in deficiency syndrome was higher than that in patients with yang deficiency of constipation(P<0.05).The initial sensory threshold of the syndrome group was higher than that of the positive group(P<0.05),the initial defecation threshold of the Yang deficiency syndrome group was higher than that of the Qi deficiency syndrome group(P<0.05),and the deficiency syndrome group was higher than the empirical group.The maximum tolerance capacity in Yang deficiency group was higher than that in qi deficiency group(P<0.05).Conclusion:The occurrence of chronic constipation is related to anorectal motility and sensory dysfunction.It may be related to anal sphincter laxity,contractile contraction and insufficient rectal propulsion,and the mechanism of rectal wall’s low sensitivity to volume stimulation and high tolerance.Constipation patients with different TCM syndromes have their own anorectal motility and sensory function. |
