Air pollutants has become one of important inducement which could cause acute exacerbation of the disease.However,most studies about the health effect of air pollution have been conducted in developed countries,there is a lack of systematic research on the association between air pollution and heart failure and acute exacerbation of chronic bronchitis in China.Until now,there was no research involved in the influence of air pollution on heart failure and acute exacerbation of chronic bronchitis in both areas.Therefor,Jinan,capital of shandong province,and Shenyang,capital of liaoning province,were selected as the research area in this study to explore the relationship between acute exacerbation of chronic bronchitis,heart failure and air pollution and compare the difference about the effect of air pollution on that two type diseases in Jinan and Shenyang from 2014 to 2017.The result of this research could seen as one for future research and a basis for the formulation of relevant national policies,and also provide suggestions and guidances for the prevention of respiratory diseases and cardiovascular and cerebrovascular diseasesHospital records of acute exacerbations of heart failure and chronic bronchitis were collected.Data of air pollutants in jinan were collected from shandong provincial department of environmental protection,and data of air pollutants in shenyang were collected from shenyang municipal environmental protection bureau,including daily average concentrations of PM2.5,PM10,SO2,NO2,O3 and CO.Data on meteorological factors:data on meteorological factors in jinan and shenyang include temperature,relative humidity and air pressure,from the national meteorological administration.Establish a connection function for the analysis of the distribution of poisson distribution delay nonlinear model air pollutant effect on the rate of disease in hospital,in the introduction of "week" model and holidays and yuan variables,the model introduced in daily average temperature,relative humidity,air pressure variable,to control the daily average temperature,relative humidity and air pressure of air pollutant concentration and the confounding effects of admission rate controlled by natural spline smoothing function nonlinear confounding factors.Stratified analysis of gender and age was established to study the impact of air pollutants on admission to hospital in different age and gender groups.The average daily concentrations of SO2,PM2.5,CO,NO2 and PM10 in jinan and shenyang are higher in spring and winter than in summer and autumn,showing obvious seasonal changes and the overall trend is u-shaped.By comparing the influence of air pollution on admission for heart failure in the two places,it was found that the admission risk of fine particulate matter in shenyang was higher than that in jinan,and the admission risk of SO2 and PM10 in shenyang was relatively weak,compared with that in jinan.The admission risk of NO2 in shenyang area is higher than that in jinan area in LagO to Lagl area.By comparing the aggravation of two air pollution to chronic bronchitis,it is found that PM10 and PM2.5 in jinan area have a great influence on the population,while PM2.5 in shenyang area is concentrated in Lag6 to Lag7 in the image of the population.PM2.5 in jinan area has a gradual decrease in the effect of Lag6 to Lag1 on the population.The effect of ozone on human population is weak.In jinan area,SO2 has a delayed effect on the crowd.In Lag5 to Lag7,SO2 has a greater impact on the crowd,while in shenyang area,SO2 has a greater impact on the crowd from Lag2 to Lag4.Stratified analysis found that SO2,CO,NO2,PM1.5 and PM10 in jinan area at LagO and Lag1 had a greater risk of admission for patients with heart failure aged 65 or above than those aged less than 65.The influence of O3 and SO2 on admission risk of heart failure was greater in men than in women in jinan area.Women with acute exacerbation of chronic bronchitis in shenyang are more sensitive to air pollutants than men.The effects of air pollution on acute exacerbation of chronic bronchitis and heart failure are different.Increased daily concentrations of particulate matter,SO2,NO2 and O3 in Jinan and Shenyang increase the risk of hospitalization for heart failure.In jinan,the admission risk of SO2,CO,NO2,PM2.5 and PM10 in Lag0 and Lagl among age≥5 is greater than that of Age<65.The influence of O3 and SO2 on admission risk of male patients was greater than that of female patients in Jinan area.PM2.5,PM10,SO2 and NO2 in Jinan and Shenyang increased the risk of acute exacerbation of chronic bronchitis.The impact of PM2.5,PM10 on people aged 65 or above is greater than that of people younger than 65.
