| Objective: Comparison of therapeutic effects between tubeless percutaneous nephrolithotomy and holmium laser lithotripsy with soft ureteroscope in the treatment of renal calculi.Methods:From March 2015 to October 2017,162 cases of renal calculi were treated with ureteral soft endoscopy combined with holmium laser lithotripsy and unmanaged percutaneous nephrolithotripsy(TPCNL)in Yaji Mountain Hospital of Southern Anhui Medical College.All cases were treated with ureteral soft endoscopy and holmium laser lithotripsy.KUB IVP was confirmed by urography and CT.The patients were divided into TPCNL group and FURS group according to the operation mode.Each group was defined as large stone group(> 20mm)and small stone group(≤ 20mm)according to stone size.The operation time,intraoperative bleeding,postoperative urinary tract infection,postoperative hospitalization days,and stone clearance rate were compared between the two groups.In March,the calculi clearance rate and complications occurred.SPSS19.0 statistical software was used to compare different groups of data,T test and analysis of measurement data,2 test analysis of count data,using P<0.05 as the difference was statistically significant.Results:There were significant differences in lithotripsy time,total operation time,Hemoglobin reduction value and postoperative hospitalization days between TPCNL group and FURS group in stone ≤ 20 mm group.There was no significant difference in stone clearance rate,postoperative bleeding rate and stone clearance rate in one month after operation(P > 0.05).In the group of stone > 20 mm,urinary tract infection was found in the two groups.The bleeding rate was significantly different from that of 3 months after operation(P < 0.05).Conclusion:Both TPCNL and FURS are effective methods for the treatment of renal calculi.The two surgical methods have their own advantages.The stone removal rate is high and the infection rate is low.The renal calculi > 20 mm in diameter are considered as the first choice for renal calculi > 20 mm,the first choice is minimally invasive,and the recovery is fast after operation.Renal calculi considered as diameter ≤ 20 mm were preferred. |
