Study On The Optimization Strategy Of Outpatient Appointment Based On The Balance Of Revenue And Resource

The optimization of outpatient appointment system is the key to increase the income,improve the utilization rate of medical resources,shorten the waiting time of patients,and improve the satisfaction of patients.However,at present,the appointment system is not perfect,the exchange of information between doctors and patients is limited,and there exists comprehensive factors such as the impact of environmental factors that should be taken into account,as a result,there is still a long way to go to optimize outpatient appointament system.Among the many factors that affect the income of the clinic,cancellation and no-show are most significant,Which Will directly lead to a lower number of outpatient patients served,the waste of doctors normal service capacity,longer waiting time for patients,doctors overtime and uneven workload for various doctors.This paper takes cancellation and no-show of patient into the research perspective in a mixed appointment system,the cumulative benefit of the clinic is described by dynamic programming and Markov process model,what’s more,the real outpatient appointment system which considers two sorts of clinic,clinic with individual doctor and multiple doctors,are simulated.According to the model,the existence of the optimal appointment strategy is proved and the corresponding algorithm is given.It is proved that with different no-show rates,there exists different optimal capacityallocation schemes for advanced patients and the same-day ones;the clinic will accept more advanced patients when considering two kinds of environmental factors,no show and cancellation,Compared with considering separately in one of the two;when taking multi-doctors into account,the optimal reservation in advance for any doctor is given;what’s more,by simulating the appointment system of two kinds of doctors,an expert and an ordinary doctor,and set the values of other parameters,different strategies for advanced patients in the reservation period for them are obtained,It can improve the overall benefit of the outpatient to implement differential pricing policies for the service of the two doctors.
