Objectives:The study based on a mobile medical platform Transitional care intervention plan formulation and implementation,a comparison of patients with type 2 diabetes self-management behavior and health outcomes and quality of life changes before and after the intervention,explore the Transitional care intervention based on mobile medical platform application effect in patients with type 2 diabetes,so as to medical personnel and related health education workers better references are provided for the management of diabetic patients,and to help people with diabetes to establish effective intervention management model to provide the reference.Methods:The study was designed with the experimental study of the self-and-back control.With convenience sampling method,according to the inclusion criteria and exclusion criteria,from May 2017 to August 2017 in Jilin province Yanji City hospital discharged from hospital after treatment in patients with type 2 diabetes,according to the actual circumstance of patient's medical records and filter meets the criteria for the patient,to meet the criteria for the patients for three months continue nursing intervention based on mobile medical platform,respectively,prior to the intervention intervention after 1 month,3 months to collect the relevant data,the data is repetitive measure analysis of variance,t test and observation of self management behavior of the two groups of patients,clinical metabolic index(FPG,2 hPBG,BMI,etc.)and quality of life change,intervention and effect evaluation,all data using spss17.0 were analyzed.Results:(1)Study subjects in general.Among the 86 patients enrolled in this study,the mean age was(56.49±9.10),and the history of diabetes was on average(8.78±5.69),with more males than females(69.8%vs30.2%).Compared with the han nationality and other ethnic minorities(58.1%vs41.9%),the average monthly income of the patients in the family was lower,with the largest proportion of RMB 3,000,accounting for 60.5%(52 people).(2)Comparison of self-management behaviors of diabetic patients before and after intervention:The score of patients' self management behavior before Transitional care intervention is(81.95±23.67),for 1 month intervention is(83.08±22.76),intervention for 3 months(84.50±21.61),the total score of self management behavior interventionin patients with prolonged gradually increase,the difference was statistically significant(P<0.05),of which six dimensions by up is blood sugar monitoring in dimension,the average increased by 3.92%,improve the least dimension for medication compliance,increased by 1.63%on average.(3)Comparison of metabolic indexes of diabetes patients before and after intervention.1 months after intervention,fasting blood sugar,2 h postprandial blood glucose,BMI,and diastolic blood pressure are improved,and had statistical significance(P<0.05),and systolic blood pressure are improved,but improve no statistical significance(P<0.05);3 months after intervention,fasting blood glucose,2 h postprandial blood glucose,BMI,systolic blood pressure and diastolic blood pressure are improved,the result was statistically significant,after intervention in 1 month and 3 months after intervention compared metabolic index,in addition,2 h postprandial blood glucose and fasting glucose,BMI improvement was statistically significant,systolic blood pressure and diastolic blood pressure also have improved,but the result has no statistical significance(P>0.05).(4)Comparison of survival quality of diabetic patients before and after intervention.Before intervention in patients with total score of quality of life is(122.44 ±38.53),1 month intervention later is(120.83 ± 36.67),intervention for 3 months is(111.90± 36.07),on average 8.61%lower,compared with before,the difference statis-tically significant(P<0.05).Four dimension scores decreased,the degree of satisfaction,influence degree and degree of anxiety related to the society,family or career three dimensions difference was statistically significant before and after the intervention,and disease related anxiety score was no difference before and after intervention(P>0.05).Conclusion:(1)Transitional care intervention based on mobile medical platform can improve self-management behavior of patients with type 2 diabetes.(2)Transitional care intervention based on mobile medical platform can improve fasting glucose,2h blood glucose and BMI after meal.(3)Transitional care intervention based on mobile medical platform can improve the quality of life of patients with type 2 diabetes.(4)The Transitional care model of diabetes based on mobile medical care is worthy to be popularized and applied,but its long-term and large sample effects need to be studied.
