| Objective The fracture of mandibular condyle(mandibular condylar,fracture,MCF)were analyzed and classified all types of fractures.Chooseing different operation methods and analyzing the advantages and disadvantages of various types of surgery and postoperative complications,thus providing the treatment of condylar fractures reference for clinicians.Methods Collected the clinical data of 22 cases(24 sides)of condylar fracture from oral and maxillofacial surgery in the First Affiliated Hospital of Anhui Medical University.Pantomography or 3D CT were taken before the operation to analyze the type of fracture of the patients and to make a reasonable plan for the treatment.Using preauricular approach and retromandibular approach during surgery process.Then reconstructed temporomandibular joint by positional screws,micro titanium plate or removing the fraction of dislocated condyle process.Taked X-ray film or CT,and analysed wound recovery,mouth opening,function of facial nerve and temporomandibular joint during the follow-up period.Result(1)The facial symmetry,occlusal function and anatomic stability of the 22 patients were significantly improved after operation.(2)The mean preoperative open mouth was(18.1±3.31)mm,and the improvement of mouth opening were improved obviously during consultation period,the difference was statistically significant(t=-20.16,P<0.05).(3)5 patients had facial nerve injury after operation.A case of zygomatic branch dysfunction occured at high fracture;Facial nerve injury in dondylar neck fracture was 3 cases(2 cases of zygomatic branch,1 case of zygomatic branch + buccal branch);A case of submandibular branch dysfunction occurred atlower fracture.The facial nerve injury were temporary in 5 patients and the facial nerve function was recovered within three months by medication.The patients were satisfied with the effect after operation.Conclusion By selecting appropriate surgical procedures for different types of condylar fractures had a satisfactory results. |
