Objective : To explore the relationship between the polymorphism of aldehyde dehydrogenase 2(ALDH2)gene and the pathogenesis of coronary heart disease in Chinese Han population.Methods: This study enrolled 505 hospitalized patients admitted for coronary angiography at the Air Force General Hospital Heart Center from May 2016 to May 2017.It included 375 confirmed CHD patients,acute coronary syndrome(ACS)338 cases,130 cases of non-CHD group.Biochemical indicators and basic information were collected from all enrolled patients,the ALDH2 gene polymorphism was analyzed by sanger sequencing,multiple risk factors including ALDH2 gene polymorphism were analyzed by logistic regression analysis in CHD patients.CHD patients were divided into PCHD(male<55 years,female<65 years)group(n=150)and non-PCHD(male ≥55 years,female ≥65 years)group(n=225),according to the number of vessel lesions,the patients were divided into single and multiple vessel lesions group.The distribution of ALDH2 genotypes in CHD group and non-CHD group,ACS group and non-CHD group,PCHD group and non-PCHD group were compared,according to the risk factors of coronary heart disease for a variety of subgroup analysis;at the same time,The distribution of ALDH2 genotype in single-vessel disease group and multi-vessel disease group was compared,Gensini score was applied to all diseased vessels,compare the Gensini scores of wild type and mutant type.According to whether the face became flushed after drinking,the enrolled patients were divided into alcohol flushing syndrome(AFS)group(n=135)and non-AFS group(n=370);According to whether used to drinking,they were divided into accustomed to drinking group(n=189)and no drinking custom group(n=316).Using SPSS22.0 for statistical analysis.Results 1.ALDH2 genotype distribution in line with the Hardy-Weinberg equilibrium.2.There was no significant difference in the distribution of ALDH2 genotype between CHD and non-CHD(P>0.05),it is also among all subgroups,logistic regression showed that ALDH2 gene was not a predisposing factor of CHD after adjusting for gender,age,smoking,drinking,BMI,hypertension,diabetes,hyperlipidemia and family history of CHD(P= 0.581,OR =1.156,95% CI: 0.692~1.930).3.The distribution of ALDH2 genotype was not significantly different between ACS group and non-CHD group,it is also among all subgroups(P>0.05,P=0.43),logistic regression showed that ALDH2 gene was not a predisposing factor of ACS after adjusting for gender,smoking,drinking,BMI,hypertension,diabetes,hyperlipidemia and family history of CHD(P=0.773,OR=0.922,95%CI: 0.532~1.600).4.There was no significant difference in the distribution of ALDH2 genotype between PCHD and non-CHD group,logistic regression showed that ALDH2 gene was not a predisposing factor of PCHD after adjusting for gender,smoking,drinking,BMI,hypertension,diabetes,hyperlipidemia and family history of CHD(P =0.729,OR =1.098,95% CI: 0.648~1.859).5.There was no significant difference in the distribution of ALDH2 genotype among different number of vessel lesions and in Gensini score between wild type and mutant group.(P>0.05).6.The incidence of ALDH2 mutant(GA+AA)was significantly higher in AFS group than in no AFS group(67.4% vs 10.5%,P<0.01).The gene mutation frequency was markedly higher in no drinking custom group than inaccustomed to drinking group(29.7% vs 19.1%,P<0.01).Conclusion: 1.The results of this study do not suggest that the ALDH2 gene polymorphism is an independent risk factor for CHD in Chinese Han population.2.The results of this study do not suggest that the ALDH2 gene polymorphism is an independent risk factor for ACS in Chinese Han population.3.The results of this study do not suggest that the ALDH2 gene polymorphism is an independent risk factor for PCHD in Chinese Han population.4.There is no significant correlation between ALDH2 gene polymorphism and the severity of coronary heart disease in Chinese Han population.5.The ALDH2 mutant is associated with AFS.AFS may predict ALDH2 mutations to some extent.
