Performance Evaluation And Research On Conprehensive Reform Of County Public Hospitals In Jiangxi Province

In China,public hospitals are the main body of medical service,and the reform of public hospitals is not directly related to the success or failure of the new reform.Public hospitals at the county level are the market leader in the rural three-level health service network and the urban and rural medical and health service system,promote the reform of public hospitals at the county level comprehensive is deepening the reform of medical health system,effectively alleviate the masses "a doctor" difficulty and high cost of key link of the problem.Public hospitals at the county level comprehensive breadth of reform,a difficult task,break with medicine in jiangxi province still need to strengthen medical achievements,need deepening management system and the personnel salary system reform,the medical service ability remains to be improved,the reform of the comprehensive and correlation remains to be enhanced,and need the problem oriented,further refined to perfect the policies and measures,continue to deepen the reform.This article first expounds the reform of public hospitals at the county level comprehensive background in domestic policy,since the 17 th national and provincial policy measures for many times,steadily promote the reform of public hospitals at the county level comprehensive from pilot to full-scale;Second,the comprehensive reform of public hospitals at the county level at home and abroad are reviewed.Some important research results of valuable,including domestic scholars LiuJing about breaking the medicine for the medical problem research,Ma Dongping comprehensive reform of public hospitals at the county level on the hospital running efficiency research as well as the influence of foreign scholars Milern Snow research on German public hospitals financing ways,etc.;Thirdly,the comprehensive reform of county public hospitals in jiangxi province was studied through literature and policy data research,expert consultation and field investigation.Based on the performance evaluation index system,the evaluation index system objectively scores the selected 10 counties(cities).The final score is 67.6,and the performance evaluation level is "qualified".In terms of target completion,patients individual pay scale according to the requirement to carry out the cancel the drug addition,the county finance supporting fund,the hospital management fee,public hospital medicine accounted for at the county level(excluding Chinese medicine yinpian)and county level public hospital outpatients time cost,at the county level public hospital discharge time of patients cost,patient satisfaction seven objective has less than 60%,especially public hospital medicine accounted for at the county level(excluding Chinese medicine yinpian)success rate is 0.Public hospitals at the county level the price mechanism,public hospital personnel system at the county level and county level public hospital medical insurance payment methods,at the county level public hospital drugs and high value medical consumables supply security system,public hospitals at the county level comprehensive supervision system,public hospitals at the county level health service capacity building aspects of the reforms have landed and achieved certain results.At the same time,the study finds that the reform progress is uneven between counties.Some county level government reform consciousness is missing,insufficient attention reform;The management level of county-level public hospitals still needs to be improved;Some county-level financial compensation has been delayed;The proportion of drugs in county-level public hospitals is still high.The cost of outpatient and inpatient patients at county-level public hospitals is still high;The funds of county-level public hospitals are still too tight;The income of medical staff in county-level public hospitals needs to be improved;At the county level,there is still some economic pressure.Next,in jiangxi province,the government can improve the assessment and performance evaluation mechanism,hierarchical diagnosis and operation mechanism,financial compensation mechanism,health service pricing mechanism and the mechanism of drug purchase price,promoting the construction of the new "3 d" linkage system,strengthen the hospital internal and external management,and other aspects,to solve the problems existing in the comprehensive reform of public hospitals at the county level.Reform of public hospitals at the county level is a long-term process,there are a lot of problems need time,and later will encounter new problems,its effect is produced by a process of quantitative change to qualitative change,at present still in the phase of quantitative change.The performance evaluation only chose 10 counties(cities),does not necessarily represent the progress of the entire jiangxi province,at the same time,the performance indicators weights questionable parts,involving the horizontal contrast reflects the effect of performance indicators.In the future work,I will continue to focus on county in jiangxi province public hospital reform policy,through continuously improve performance indicators,the implementation of more scientific and more comprehensive performance evaluation,strive for public hospital management department at the county level to provide more valuable reference basis.
