Background and purposeCoronary atherosclerotic heart disease constitutes a major cause of death worldwide and is a serious threat to human health.Inflammation plays an important role in coronary artery stenosis.The high levels of serum IL-J6,as a pro-inflammatory cytokine,related to patients with coronary atherosclerotic heart disease.The aim of the study was to investigate the relationship between the serum IL-16 levels and coronary artery stenosis and left ventricular ejection fraction(LVEF)in coronary artery disease.MethodsSeventy-eight patients with different degrees of coronary heart disease diagnosed by coronary angiography were selected for the study.All subjects were divided into different degree coronary stenosis groups by Gensini scores,and medical history records and clinical examination indexes were collected.The serum IL-16 levels were measured by ELISA kit and LVEF values measured by echocardiography.Univariate and multivariate analysis were used in the data analisis to prove the association between serum IL-16 with LVEF and coronary artery stenosis.A generalized linear model was used to predict the degree of coronary artery stenosis.ResultsThe Gensini scores were divided into four groups:0 score for G1 group,1-25 scores for G2 group,26-61 scores for G3 group and 61 and over scores for G4 group,which indicated different degrees of coronary stenosis.Univariate analysis showed the differences of age,sex,hypertension,diabetes,smoking,BMI(Body Mass Index),LDL-C,high-density lipoprotein cholesterol(HDL-C),triglyceride(TG),total cholesterol(TC)and Homocysteine(Hcy)were not statistical significant(P>0.05);However,the mean values of IL-16 and LVEF levels were statistical significant among the four groups(P<0.05).With the severity degree of coronary artery stenosis increasing,the expression levels of IL-16 increased(P<0.05),while the LVEF levels decreasedsignificantly with a linear trend(P<0.05).The binary Logistic analysis showed that after adjusted the effects of age,gender,hypertension,diabetes,smoking and blood lipids,the serum levels of IL-16 were associated with the severity of coronary stenosis,which indicated its was an independent risk factor for the disease(OR=1.014,95%CI=1.003-1.026,P=0.017).There was a negative correlation between the serum IL-16 levels and the LVEF levels.A linear regression equation was significantly established for the two variables.The determination coefficient(R2)was 0.14 for the equation.The generalized linear regression model was established by the combination of the serum IL-16 with other risk factors of traditional epidemiology to predict the Gensini score which related to the degree of coronary stenosis.The area of ROCcurve(AUC)is =0.825,its 95%CI=0.735-0.915,P<0.001.The sensitivity and specificity of the diagnosis of coronary artery stenosis were 65.1%and 80.0%respectively,which reached a moderate level of auxiliary diagnosis.ConclusionThis study found that the elevated serum level of IL-16 was positively correlated with the severity of coronary stenosis and was an independent risk factors for the disease.The serum IL-16 levels were negatively correlated with LVEF levels and both of them associated with coronary stenosis;A generalized linear model combining serum IL-16 levels and other risk factors of the disease to predict the Gensini scores,which can aid in the diagnosis of coronary stenosis.
