Study On The Normal Reference Range Of Thyroid Function In Pregnant Women And Related Discussions

Objective To discuss the function changes of thyroid during pregnancy and the trend of changes in serum index.The Thyroid function of pregnancy stage specificity index: promote thyroxine(TSH)and free thyroxine(FT4)reference range;Discussion of Thyroid peroxidase antibodies(TPOAb)positive effect on the related hormone levels;Select a reasonable screening group for thyroid diseases.Methods Selection in June 2016 to December 2016 in our hospital thyroid function tests of pregnancy women and normal non pregnancy women as study group,in accordance with the criteria for the record after thyroid function index of TSH,FT4 values.Established in accordance with the guidelines given in the reference standard,the method of using data distribution with reference to the 2.5 percentile threshold,97.5 with reference to the percentile of upper limit,set up the lab thyroid function index TSH,FT4 pregnancy specific reference range;The influence of TPOAb positive on the thyroid index of pregnant women was analyzed by comparing the indicators of positive group and observation group.At the same time,the specific reference range of this experiment was used to evaluate the thyroid function of pregnant women.Results The range of values of pregnancy-specific thyroid function in early,middle and late stages were TSH(0.06-5.74 m IU / L,0.10-5.30 m IU / L,0.53-4.67 m IU / L),FT4(8.29-15.28 pmol / L,6.56-11.97 pmol / L,5.79-10.30 pmol / L).Serum TSH and FT4 levels showed a weak negative correlation throughout pregnancy.Thyroid function was assessed in pregnant women in this study using the established reference range.Overall screening showed a 0.62% incidence of Overt hypothyroidism(OH)and a 3.29% incidence of Subclinical hypothyroidism(SCH),The incidence of hypothyroxinemia 5.90%;only the positive group found that the incidence of three diseases were 1.33%,5.33%,7%;the observation group found that the incidence of three diseases was 0.46%,2.83%,5.65%.72.15% of patients with hypothyroidism will miss screening for gestational groups at risk.Conclusion 1.To establish pregnancy-specific thyroid function index TSH,FT4 reference range.2.The range of reference values obtained in this study is different from the range recommended in the guideline,and differs from the reference range set up in other regions of the country.It makes sense to establish the reference range in this region.3.The risk of hypothyroidism in TPOAb-positive pregnant women is much higher than that in TPOAb-negative pregnant women,while there is no significant correlation between serum TSH and FT4 levels in TPOAb-positive pregnant women.4.Only high-risk factors for pregnant women with thyroid function screening missed the high rate of missed diagnosis,recommended for all pregnant women screening strategy...
