ObjectivesUsing echocardiography techniques to investigate the prevalence of pulmonary arterial hypertension(PAH)in systemic lupus erythematosus(SLE)patients,and to estimate the risk factors for PAH in patients with SLE.MethodsFrom January 2011 to December 2015,medical records of SLE patients in the First Affiliated Hospital of Anhui Medical University and Anhui Provincial Hospital were collected.Pulmonary arterial pressure was measured by resting transthoracic echocardiography(TTE),PAH was defined as a systolic PAP(PASP)> 30 mm Hg.And also need to rule out other connective tissue diseases(a PL syndrome,left heart disease,valvular heart disease,congenital heart disease,HIV and portal hypertension).Moreover,potential risk factors for SLE patients with PAH(thrombotic factors,clinical manifestations of SLE,laboratory tests,and disease activity)were analyzed.Results 1639 patients with SLE were included in the study,and 1499(91%)patients were women,95(5.8%)patients were PAH.After the analysis of the risk factors of SLE patients with PAH,we found that the prevalence of high fibrinogen in SLE patients with PAH was significantly higher than that in the SLE patients without PAH(P=0.021);the incidence of serositis in SLE patients with PAH was markedly higher than that in SLE patients without PAH(P<0.001);SLE patients with PAH had higher rate of thrombocytopenia(P=0.011).Moreover,the positive rate of anti-SM antibody in SLE patients with PAH was lower than that in SLE patients without PAH(P=0.039).On the basis of univariate analysis,multivariate logistic regression analysis showed that high fibrinogen(OR=1.600,95%CI: 1.039-2.463),serositis(OR=2.754,95%CI:1.753-4.327)and thrombocytopenia(OR=1.801,95%CI:1.137-2.852)were risk factors for PAH in SLE patients.Conclusions In our study,the prevalence of PAH in SLE patients was 5.8%.High fibrinogen,serositis and thrombocytopenia were significantly associated with PAH in SLE patients.The results suggest that hypercoagulable state,organ damage and hematological abnormalities may be closely related with PAH in SLE patients.Objectives To assess the prevalence of serositis in systemic lupus erythematosus(SLE)patients,and to examined the risk factors for serositis with SLE.Methods From January 2011 to December 2015,medical records of SLE patients in First Affiliated Hospital of Anhui Medical University and Anhui Provincial Hospital were collected.Serositis was diagnosed when the patient appears symptoms of pleurisy or pericarditis.In the diagnosis of SLE-related serositis,the patients with malignant,thromboembolic and exudative reasons(such as hypoalbuminemia and heart failure)were excluded.Potential risk factors for serositis in patients with SLE,including thrombotic factors,clinical manifestations of SLE patients,laboratory tests,and disease activity were analyzed.Results 1668 SLE patients were included in the present study,1526(91.5%)were women,and 298(16.7%)had serositis.The results of multivariate logistic regression analysis showed that fever(?38 ° C),active stage of lupus disease,high D-dimer,low C4 and high ESR were the risk factors for serositis in SLE patients(P <0.05);fever(?38 ° C).Active lupus disease and high D dimer and men were the risk factors for pleuritis in SLE patients(P <0.05);fever 38 ° C).Active lupus disease,high D dimer,low C4 and high ESR were risk factors for pericarditis(P<0.05).The results of ordinal logistic regression analysis showed that fever(?38 ° C),active lupus disease and high D dimers were significantly associated with the progression of serositis(P=0.003;P<0.001;P<0.001).Conclusions In our study,the prevalence of serositis in SLE patients was 17.9%.Active SLE and high D-dimers are associated with serositis.Active SLE,high D-dimers and gender(man)were associated with pleuritis in SLE patients.Mareover,active SLE,high D-dimers,low C4 and high ESR were associated with pericarditis in SLE patients.
