[Objective]To prove the changes of myocardial enzymes including cardiac troponin(cTn),serum b-type natriuretic peptide(BNP),heart type fatty acid binding protein(H-FABP),soluble ST2(sST2)and their significance in infant pneumonia complicated with heart failure.[Methods]Fifty 29 days to 3 years old children with pneumonia complicated with acute heart failure were chosen in Taizhou People’s Hospital,Pediatric department during 2013.01~2016.12,fifty cases of uncomplicated pneumonia were randomly selected.All cases were diagnosed according to standards,improved ROSS score method was applied to heart failure classification.Fifty cases of healthy children in control group were randomly selected from the Department of Children’s Health Prevention Physical Examination of the same age.General information was collected.CTn were measured with ILMA.The level of NT-pro-BNP,H-FABP,sST2,etc.were detected with enzyme-linked immunosorbent assay kit.SPSS 11.5 software package was used for data analysis.[Results]1,Serum myocardial enzyme test results:The serum myocardial enzyme indicators AST、LDH、CK、CK-MB、α-HBDH between the pneumonia with heart failure group、pure pneumonia group and control group were significantly different(P<0.05)before treatment.But there was no statistical difference(P>0.05)in all serum myocardial enzyme indicators after treatment except for LDH.Myocardial enzyme in pneumonia with heart failure group had statistical significance(P<0.05)before and after treatment.Serum myocardial enzyme between different rank of heart failure in the level of had statistical significance(P<0.05).Only CK-MB had statistical significance(P<0.05)through pairwise comparison in all groups.2,Pneumonia with heart failure,pure pneumonia and control group before treatment cTn,BNP,H-FABP,sST2 and other myocardial injury markers were statistically different between different groups(P<0.05).Before trentment,Cardiac markers of cTnI、BNP、H-FABP and sST2 etc,were statistically different(P<0.05)between different groups.Comparing two groups,only cTnI,sST2 had no statistical difference between pure pneumonia group and control group(P>0.05),and other situations were statistically different(P<0.05).Only the BNP levels had statistically difference(P<0.05)through pairwise comparison after treatment.Pneumonia with heart failure group before and after treatment cTnI,BNP,H-FABP,sST2 level difference had statistical significance(P<0.05),serum cTnI,BNP,H-FABP and sST2 level between different level of heart failure were statistically different(P<0.05);Only sST2 level pairwise comparison between different level of heart failure group were statistically different(P<0.05).[Conclusion]1,Comparison of pre-and post-treatment,myocardial enzymology in pneumonia with heart failure group had statistical difference and different levels of heart failure had statistical significance,in terms of grade diagnosis of heart failure,CK-MB has high sensitivity and specificity.2,Serum cTnI,BNP,H-FABP and children sST2 levels before and after treatment in the pneumonia with heart failure group had statistical difference and different level of heart failure also had statistical difference,comprehensive analysis,in terms of hierarchical diagnosis of heart failure,an sST2 sensitivity,high specificity.
