Analysis The Difference Of Right Heart Hemodynamic,Functional And Erythrocyte Sedimentation Rate Between Simple Pneumoconiosis And Silicosis Complicated By Tuberculosis

Objective To explore the difference of right heart hemodynamic,functionalanderythrocytesedimentationratebetweenthesimple pneumoconiosis patients and the silicosis complicated by tuberculosis(SCTB)patients,and research whether it has clinical significance.Methods We selected78 cases of males patients with pneumoconiosis by used simple random sampling method and divided them into two groups,the simple pneumoconiosis group(41 cases)and the SCTB group(37 cases)according to who had pulmonary tuberculosis or not.34 males were chose as the control group.Collecting the general data of the study subjects and calculating the body mass index(BMI).Measured the right ventricular outflow tract diameter(DRVOT),the main pulmonary artery diameter(DMPA),the blood flow velocity of pulmonary artery valve(VPA),the blood flow velocity of right ventricular outflow tract(VRVOT),the velocity time integral of right ventricular outflow tract(TVIRVOT),three tricuspid regurgitation velocity(VTR),tricuspid annular plane systolic excursion(TAPSE),isovolumetric contraction time(ICT)of right ventricular,isovolumetric relaxation time(IRT)and ejection time(ET)by echocardiography.Pulmonary artery systolic pressure(PASP),pulmonary vascular resistance(PVR)and Tei index of right ventricular(RV-Tei)were calculated according to the formula.Blood text index included erythrocyte sedimentation rate(ESR),hematocrit(HCT)and platelet aggregation(PCT).SPSS 21.0 statistical software was used for statistical analysis.Result(1)The BMI was reduced in the simple pneumoconiosis group and the SCTB group compared with the control group(P<0.05),but there was no significant difference between the simple pneumoconiosis group and the SCTB group(P>0.05).(2)the time of dust exposure in the SCTB group was longer than the the simple pneumoconiosis group(P<0.05).(3)DRVOTVOT in the simple pneumoconiosis group and the SCTB group were higher than the control group(P<0.05),and there was no significant difference between the simple pneumoconiosis group and the SCTB group(P>0.05).(4)PASP in the simple pneumoconiosis,the SCTB group were higher than the control group,and the SCTB group was higher than the simple pneumoconiosis group(P<0.05).(5)PVR in the simple pneumoconiosis group and the SCTB group were higher than the control group(P<0.05),but there was no statistical sense difference between the simple pneumoconiosis group and the SCTB group(P>0.05).(6)VPA,VRVOTVOT in the simple pneumoconiosis group,the SCTB group were lower than the control group(P<0.05),but the difference has not statistically significant between the pneumoconiosis group and the SCTB group(P>0.05).(7)TAPSE in the simple pneumoconiosis group,the SCTB group was less than the control group,and the SCTB group was less than the simple pneumoconiosis group(P<0.05).(8)Compared with the control group,the right ventricular RV-Tei in the simple pneumoconiosis group and the SCTB group were significantly higher,and the SCTB group was higher than that in simple pneumoconiosis group(P<0.05).(9)HCT in the simple pneumoconiosis group,the SCTB group was higher than the control group,and the SCTB group was lower than that of simple pneumoconiosis group(P<0.05).(10)PCT in the simple pneumoconiosis group,the SCTB group were higher thanthe control group(P<0.05),but there was no statistically significant difference between the simple pneumoconiosis group and the SCTB group(P>0.05).(11)ESR in the SCTB group was higher than the simple pneumoconiosis group and the control group(P<0.05),but compare the simple pneumoconiosis group with the control group,the difference was not significant(P>0.05).(12)ESR was positively correlated with PASP and RV-Tei(r=0.36,P<0.05;r=0.31,P<0.05),and negatively correlated with TAPSE(r=-0.29,P<0.05).Conclusion The right ventricular function in patients with pneumoconiosis whether complicated by tuberculosis or not,which compared with healthy adults was decreased,especially in the SCTB patients.The right heart function of patients with pneumoconiosis decreased with the increase of erythrocyte sedimentation rate.All In all,there are practical significance to survey the function of the right ventricular and ESR in SCTB patients in the course of treatment.
