Objective:This study was to explore the effective method of treatment of early DF peripheral vascular disease by observing the Divaricate Gynura(L.)DC compound treatment of DF(Wagner 0,Wagner 1,Wagner 2)the curative effect of vascular disease,to prevent DF deteriorate further,to promote the healing of ulcer DF patients,to reduce the morbidity and mortality of DF and to provide the scientific basis for the clinical application of the compound.Methods:62 patients with diabetic foot who met inclusion criteria were randomly divided into research group and the control group.Two groups all use conventional western medicine therapy(such as keep blood glucose under control,antiplatelet,lower blood lipids,anti-infection and routine surgical treatment,etc.),The research team take Divaricate Gynura(L.)DC combination therapy in the treatment on the basis of the above.After 3 course of treatment(4 weeks for 1 course of treatment),Comparing before and after treatment in patients with the changes of clinical symptoms and signs,blood glucose,(fasting glucose FBG and 2h postprandial plasma glucose PBG),glycosylated hemoglobin(HbA1c),blood rheology,ankle brachial index(ABI),limb dorsalis pedis artery color doppler ultrasound.And clearing security checking the liver and kidney function.Results:1.The general situation: The differences of Gender,age,duration of diabetes,diabetic foot disease progression,the Wagner lesion grading,diabetic foot ulcer area of two groups are no statistical difference(P>0.05).2.Blood glucose:Compared with before treatment,two groups of FBG,PBG,HbA1 c were very significantly decreased(P<0.01).After treatment group compared with control group,FBG of difference is statistically significant(P<0.05),but PBG and HbA1 c no statistically significant difference(P>0.05);3.Blood rheology:Compared with before treatment,two groups of whole blood low shear viscosity value,whole blood viscosity,blood sedimentation equation K value,erythrocyte aggregation index were very significantly improved(P<0.01).After treatment group compared with control group,whole blood low shear viscosity value,whole blood viscosity,erythrocyte aggregation index,blood sedimentation equation K value differences were statistically significant(P<0.01);4:ABI and colour duplex ultrasonography of limb dorsalis pedis artery:Compared with before treatment,the control of ABI,shrinkage peak period of blood flow velocity,early diastolic reverse maximum blood flow velocity and diastolic forward the improvement of the blood flow velocity statistically significant(P< 0.01).Dorsalis pedis artery diameter to improve no statistical significance(P> 0.05),and the team’s ankle brachial index,shrinkage periods peak blood flow velocity,early diastolic reverse maximum blood flow velocity and diastolic forward dorsalis pedis artery blood flow velocity and inner diameter to improve had statistical significance(P<0.01),After treatment group compared with control group,ankle brachial index,shrinkage peak period of blood flow velocity,early diastolic reverse maximum blood flow velocity and diastolic forward blood flow velocity,the differences between the dorsalis pedis artery diameter were statistically significant(P<0.01);5.Signs/symptoms integral to change and improve curative effect:Compared with before treatment,two groups of symptoms/signs integral reduced statistically significant(P<0.01),treatment group compared with control group,the symptoms/signs integral differences statistically significant(P<0.05),the team with the control of diabetes systemic symptom effectiveness were 87.5%,87.5%,the team with the control group in diabetic foot local symptoms/signs effectiveness were 96.8%,96.8%,the cure rate was 28.1%,13.3%,differences between the two groups was statistically significant(P<0.05).Conclusion:The Divaricate Gynura(L.)DC compound not only can improve the patients’ clinical symptoms and signs,also can improve the blood rheology,ABI,the function of the dorsalis pedis artery.That shows this compound has therapeutic effect on diabetic foot vascular lesions.This compound can be further research and development,and is worth clinical promotion.
