Objective: To determine the value of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA)in the pulmonary and/or m ediastinal diseases.Method: The clinical data of 142 patients who underw ent EBUS-TBNA or c TBNA between January 2014 and June 2017 were re trospectively reviewed.The pathological results,diagnostic yield,sensitivity,specificity,positive predictive value,negative predictive value and accura cy of this procedure were statistical analysis.Results: EBUS-TBNA from77 patients was successfully performed to obtain samples from 158 station s of lymph nodes,and the diagnostic yield,sensitivity,specificity,accurac y,positive predictive value and negative predictive value were 65.8%,86.1%,100%,87.0%,100% and 33.3%.c TBNA from 65 patients was succes sfully performed to obtain samples from 134 stations of lymph nodes,and the diagnostic yield,sensitivity,specificity,accuracy,positive predictive v alue and negative predictive value were 41.8%,60.7%,100%,63.1%,100% and 14.3%.All patients had no serious complications.Multivariate log istic regression analysis show that lesion short diameter,puncture times an d the types of puncture may be the independent predictive factors of path ological positive rate incidence.Conclusion: EBUS-TBNA has higher sensi tivity,specificity and accuracy in diagnosis of pulmonary and/or mediastin al diseases,and it is an significant diagnosis method. |
