Objective:To study the relationship between Mycoplasma pneumoniae pneumonia severity in children and plasma D-dimer level so as to provide a fast and reliable predictive index and guide the rational use of drugs in clinic.Methods:This study collected 120 children with Mycoplasma pneumoniae pneumonia hospitalized in the two family of Bethune First Hospital of Jilin University from June 2017 to December 2017 as the research object.The diagnosis of Mycoplasma pneumoniae pneumonia was based on the diagnostic criteria of Mycoplasma pneumoniae pneumonia in the eighth edition of "Zhu Futang Practical Pediatrics".patients were divided into A group(plasma D-dimer >500 ug/L,n=60)and B group(plasma D-dimer< 500 ug/L,n=60).They received the same routine treatment,including anti Mycoplasma pneumoniae,phlegm,fluid retention,electrolyte balance and symptomatic treatment,and so on.We compared the heat peak,physical signs,CRP,LDH,bronchoscopy,imaging findings,pulmonary complications and the incidence of extrapulmonarycomplications between the two groups.In order to further clarify the relationship between D-dimer and Mycoplasma pneumoniae pneumonia,we also conducted a correlation analysis between the heat peak,C reactive protein(CRP),lactate dehydrogenase(LDH)and D-dimer.Results:(1)Among the 120 children with Mycoplasma pneumoniae pneumonia,the level of D-dimer was obviously higher in the A group than that in B group.(2)Children in A group,when compared with children in B group,had significant higher incidence of pulmonary consolidation(18 cases)and atelectasis(9 cases).(3)The incidence of extrapulmonary complications such as rash(n=1),encephalitis(n=1)and liver function damage(n=2)in A group was also higher than B group.(4)The plasma D-dimer and inflammatory indexes,such as heat peak,white blood cell count,the number of inflammatory markers,were obviously elevated in A group.What’s more,plasma D-dimer level had positive correlation with neutrophil proportion,CRP as well as LDH.Conclusions:1,plasma D-dimers level in children with Mycoplasma pneumoniae pneumonia was significantly elevated.2,plasma D-dimer level in children with Mycoplasma pneumoniae pneumonia is positively correlated with the number of hot peaks,CRP,WBC counts,neutrophil proportion and LDH.3,plasma level of D-dimer was suitable as an indicator of assessing Mycoplasma pneumoniae pneumonia severity in children.When the level of D-dimer was higher than 1792 ug/L,patients will be diagnosed with severe mycoplasma pneumonia.
