ObjectiveTo observe the effect of intraaortic balloon pump(IABP)on the short-term prognosis of high-risk acute myocardial infarction patients undergoing emergency PCI.MethodsThe clinical data of 76 patients with high-risk AMI treated with IABP support were retrospectively analyzed from December 2012 to November 2017 in Jilin University’s China-Japan Fellowship of IABP.According to the timing of IABP,they were divided into preventive IABP group(preventive use of IABP(n=28)and remedial IABP groups(remedial application of IABP,n=48).Observation indexes: the number of coronary artery lesions in the two groups,the ratio of lesions involving the left main artery or three branches,the number of implanted stents,and malignant arrhythmia after PCI,cardiac death,all-cause mortality,and salvage use rate.Data was processed using SPSS 19.0 statistical software.The measurement data were first verified for normality,in line with the normal distribution with the mean ± standard deviation(±S),using the t-test;non-normally distributed data with a median(quartile)[M(P25,P75)] Said that the use of non-parametric test;Count data use cases and percentage of said,the rate of comparison using Chi-square(χ2)test,hierarchical group data using rank sum test,all the comparison data to P less than 0.05 as the difference was statistically significant.ResultsCompared with the remedial use of IABP group,the number of coronary lesions in the prophylactic IABP group,the incidence of left main artery or three lesions in the IABP group,the number of coronary artery stents implanted,and the Gensini score were higher.All were less than 0.05,and the difference was statistically significant.In addition,in the incidence of malignant arrhythmia after PCI,the preventive IABP group(28.57%)was lower than the rescue IABP group(58.33%)(χ2=6.283,P=0.012);in the incidence of cardiogenic shock The remedial use of IABP group(46.43%)was higher than that of preventive application of IABP group(93.75%),the difference was statistically significant(χ2=21.909,P=0.000);in the incidence of cardiac death,the preventive use of IABP patients(14.29%)was lower than the rescue IABP group(27.08%),but the difference was not statistically significant(P=0.179).ConclusionsEarly detection of high-risk patients with acute myocardial infarction and prophylactic use of IABP before PCI can reduce the incidence of adverse events such as malignant arrhythmia and cardiogenic shock after coronary intervention in high-risk AMI patients.PCI Preoperative prophylactic use of IABP has reduced the trend of cardiac death during hospitalization in patients with high-risk acute myocardial infarction,but the difference was not statistically significant(small sample size).
