Objective:The relationship between the neovascularization in the carotid atherosclerotic plaque and the ischemic cerebrovascular disease was compared by using the superb microvascular imaging.Methods:From January 2017 to October 2017 in our hospital clinic or hospitalized patients,clinically diagnosed as cerebral infarction(CI)and(or)transient ischemic attack(TIA)and carotid atherosclerotic plaques in patients with isolated carotid atherosclerotic plaque patients.The carotid plaque [18] is defined as: the intima-media thickness(IMT)? 1.5mm,and the limitations of the convex to the lumen.100 cases of carotid atherosclerotic plaques were detected by conventional ultrasound and Doppler ultrasound.243 plaques were detected.100 hypoechoic,isoechoic or heterogeneous echo patches were selected according to the inclusion criteria.The selected patients were divided into two groups,50 cases in each group,group A: a clinical diagnosis of cerebral infarction and(or)transient ischemic attack and presence of carotid plaque,plaque thickness: 1.50mm-6.10 mm,the average thickness of(3.34 ± 0.93)mm,group B: simple carotid atheromatous plaques,plaque thickness : 2.20mm-5.60 mm.The average thickness was(3.49 ± 0.79)mm.Among them,there were 78 males and 22 females,with an average age of 31-86 years and an average of(63.65±0.56)years.The sex and age of the patients were not statistically significant.General ultrasound examination on the patients,from the beginning of common carotid artery,blood vessels along the scan,extends through the bifurcation of carotid artery(carotid bulb),and continue to scan the internal and external carotid artery,transverse and longitudinal observe the presence of carotid plaque,and vascular in short axis view the thickest and most clearly observed at the carotid atherosclerotic plaque,plaque thickness and echo records.The SMI mode was used to detect the carotid artery IPN.Two patients in group A and B were observed two groups of patients respectively,and the number of IPN in the carotid artery was recorded.The data were graded according to the grading standard,and the data were statistically analyzed.According to the scoring standard of SMI,two groups of A and B patients were graded.SPSS21.0 statistical software was applied to analyze the statistical results by x2 test,p < 0.05 had statistical significance;an analysis of the consistency of the statistical results of two a and b doctors using Kappa test,Kappa > 0 expressed meaning,Kappa greater consistency and better,Kappa is greater than 0.75,said quite satisfactory consistency,Kappa < 0.4,consistency is not ideal.Kappa = 1,indicating that the two test results are exactly the same.Kappa =-1 indicates two test results are totally inconsistent;Kappa = 0,which means two test results are caused by opportunities;Kappa < 0 has no meaning in practical application.Results:Superb microvascular imaging test results:1.Group A score: 0 points: 3 cases(6%),1 points: 23 cases(46%),2 points: 24 cases(48%).Group B score: plaque thickness: 2.20mm-5.60 mm,0 points: 6 cases(12%),1 points: 35 cases(70%),2: 9(18%).2.A total of 0-1 scores were scored in 67 cases,of which 26 were in group A and 41 in group B.33 patients with a score of 2 points,of which 24 were in group A and 9 in group B.3.The detection results of two physicians in group A and B in two groups were as follows: A: 0 points: 9 cases,1 points: 58 cases,2 points: 33 cases;B: 0 points: 7 cases,1 points: 1 cases,1 cases: case report.4.By using SPSS21.0 to analyze the data using x~2 test,the number of carotid artery IPN in group A and IPN number of carotid artery in group B were compared,p < 0.05,there was statistical significance.5.Two physicians in group A and B had a consistent Kappa test for two groups of patients,Kappa = 0.801 > 0.75,indicating that two physicians of group A and B had fairly satisfactory agreement on SMI grading of neovascularization of carotid artery plaque.Conclusion:1.The number of neovascularization in the carotid artery in patients with cerebral infarction and(or)transient ischemic attack and carotid atherosclerotic plaque is more than that of those with carotid atheromatous plaque.2.The higher the SMI score,the higher the incidence of CI or TIA in patients.3.Superb microvascular imaging is a relatively new ultrasound technology.It has obvious advantages in the prediction of neovascularization in atherosclerotic plaques,and its effect is consistent with the effect of contrast-enhanced ultrasound.
