Objective: To evaluate the urinary Continence of ileal neobladder after radical cystectomy based on the relevant research of urodynamics.Methods: From 2012 to 2016,83 cases were treated with laparoscopic radical cystectomy and ileal neobladder operation in our hospital.According to some relevant exclusionary criteria,the data of 30 cases were collected,including 20 cases of males and 10 cases of females.The data included related indicators of electrolyte,routine urine tests,renal function and postoperative urodynamics.With the exclusion of the same exclusionary criteria,the data of urodynamics of 30 patients with prostate cancer who were treated with laparoscopic radical prostatectomy were collected and compared with the former.Results: In this study,the data of 30 cases of postoperative bladder cancer patients were successfully collected: follow-up visits lasted for 12-63 months,with an average of 32 months,and there were 20 cases of men and 10 of women.At the same time,the data on 30 cases of postoperative prostate cancer were also successfully collected: the follow-up duration is 13-49 months,and the average duration is 28 months.According to statistical analysis,there is no statistical difference between bladder cancer group and prostate cancer group in the following aspects: age(68.47±6.47 VS 70.3±4.75 P=0.16)Average flow rate(Qave)(8.43±4.11 VS 8.56±4.25 P=0.912),the Maximum flow rate(Qmax)(11.79±7.31 VS 11.5±5.25 P=0.530),and the Bladder compliance(BC)(25.03±12.01 VS 22.61±10.79 P=0.460).However,statistical difference between the two groups lies in the following aspects: the Post void residual(PVR)(171.4±238.95 VS 10.83±9.96 P<0.001),First desire to void(FD)(343.57±188.51 VS 230.83±119.6 P=0.002),the maximum detrusor pressure in storage period(30.73±13.29 VS 11.43±8.47 P<0.001)the Maximum cystometric capacity(MCC)(525.8±202.68 VS 367.65±132.21 P<0.001).There is no statistical difference between the female and male groups in the bladder cancer category: First desire to void(FD)(302.60 + 125.47 VS 425.50 + 264.73 P=0.367),Average flow rate(Qave)(8.50±4.87 VS 8.29±2.13 P=0.874),the Maximum flow rate(Qmax)(12.74±8.3 VS 9.90±4.53 P=0.676),Bladder compliance(BC)(24.67±11.58 VS 25.74±13.45 P=0.826)and the maximum detrusor pressure in storage period(30.90±14.25 VS 30.40±11.83).However,there are statistical differences between the two groups in the following aspects: the Post void residual(PVR)(87.50±108.82 VS 339.20±334.73 P=0.043)and the Maximum cystometric capacity(MCC)(449.25±113.3 VS 678.90±257.3 P=0.025).It is obvious that the numerical value of female group is much higher in both the Post void residual(PVR)and the Maximum cystometric capacity(MCC).There is no statistical difference for the bladder cancer group before and after operation in the following aspects: urinary leukocytes(348.6±1343.99 VS 208.5±373 P=0.989),blood potassium(4.24±0.54 VS 4.33±0.59 P=0.208)and blood sodium(139.63±3.09 VS 140±2.79 P=0.578).However,there are statistical differences in the following two aspects: creatinine(82.73±35.94 VS 89.14±20.77 P=0.007)and blood chlorine(102.67±4.14 VS 104.98±2.94 P<0.005).The data suggests that both the blood chlorine and creatinine witnessed slight increases after operation,but the indicators are still in the normal range.and there is no functional renal damage.Conclusion: The reconstructed ileal neobladder with laparoscopic radical cystectomy has a satisfying function of urine storage and urination。...
