Objective Telomerase activity is a commonly known tumor marker with certain specificity.It has activity in many malignant diseases and has a certain clinical diagnostic value.Based on the characteristics of telomerase activity,many scholars have conducted in-depth research on it,and there are many reports on the use of telomerase activity assays in the diagnosis of benign and malignant pleural effusions.However,pathology is still used clinically.In the gold standard,telomerase activity alone was not used as a diagnostic tool.The purpose of this study is to combine the results of numerous studies to investigate the diagnostic efficacy of telomerase activity in the diagnosis of pleural effusions and to evaluate whether this test can be used alone for the clinical diagnosis of malignant pleural effusions.Methods Based on the research literature on the relationship between the detection of telomerase activity and the diagnosis of pleural effusion published by a large-scale database at home and abroad in the past 15 years using a common search tool,document inclusion and exclusion criteria,screening and exclusion of unavailable data,integration and evaluation of the included study data,use of Meta-Disc/Revman statistical software for diagnostic meta-analysis.draw the corresponding forest plot and ROC curve,in order to explore the diagnostic efficiency of the diagnostic method and draw conclusions.Results Twenty-two independent studies were included in the meta-analysis according to the literature screening criteria.Data were collected and analyzed using Meta-DiSc/Revman statistical software.The results showed that the combined sensitivity(Sen)is 0.83(95%CI:0.80 to 0.85);The combined total specificity(Spe)is 0.91(95%CI:0.89-0.93);the combined positive likelihood ratio(P-LR)is 7.94(95%CI:5.65-11.16),and the combined negative likelihood ratio(N-LR)is 0,20(95%CI:0.15-0.26);the combined diagnostic odds ratio(DOR)is 44.98>1(95%CI:27.72-73.00);the mean value of the maximum intersection of sensitivity and specificity is 0.8672,and the area under the curve(AUC)is 0.9317;The data obtained from data analysis showed that the detection of telomerase activity in pleural effusions was highly effective in the diagnosis of malignant pleural effusions.Conclusions Combined with this statistical analysis,telomerase activity assay has high sensitivity and specificity in the diagnosis of malignant pleural effusion.It can be used as an important indicator to identify the nature of pleural effusion and diagnose malignant pleural effusion,but related research Show that the detection method is still possible false negatives,it is not enough as a single diagnostic indicator for diagnosis of malignant pleural effusion.
