Objective : To explore the security and effectiveness of laparoscopic pancreaticoduodenectomy(LPD).Methods : Perioperative clinical data of 45 consecutive pancreaticoduodenectomy(PD).The center of Digestive and Bloodvessle Surgery of The First Affiliated Hospital of Xinjiang Medical University were retrospective analyzed,among which patients underwent LPD(LPD group)while 25 patients received open pancreaticoduodenectomy(OPD group).Perioperative outcomes were compared within the two groups.Results:The LPD group had significantly longer operative time [(530±50)min vs(420±55)min,P <0.001],reduced estimated blood loss [(270±200)ml vs(350±300)ml,P = 0.018],and shorter length of hospital stay and intensive care unit stay [(31.5±2.3)d vs(37.3±2.7)d,P < 0.001]and[(1.2±0.9)d vs(2.1±0.7)d,P < 0.001] respectively,lower wound infection rate[(5% vs16%),P<0.001] compared to OPD group.There were no significant differences between the two groups in Time of starting activity,First flatus time,time of pulling out the gastrointestinal decompression tube,pancreatic fistula,biliary leakage,delayed gastric emptying,reoperation rate,overall morbidity rate,mortality,tumor sizes,R0 ratio,the number of harvested lymph node,the number of positive lymph node.Contrastive conclusion: Although LPD is more complicate and difficult than OPD,for veteran surgeons,LPD is also safe and effective.
