Tracer Effect Of New Nano-carbon On Pelvic Lymph Nodes In Laparoscopic Radical Cystectomy

Objective: Comparing the surgical results using new carbon nanoparticles suspension injection or not in lymph node dissection of laparoscopic radical cystectomy to provide basis of evidence-based medicine for precision surgery.Methods: To collect perioperative medical records of patients underwent radical cystectomy who were admitted to the Second Department of Urology in Jilin University First Hospital between June 2016 and December 2017.Forty-four patients were divided into two groups: tracer group and control group.Forty-eight hours before operation,twenty-two patients in tracer group underwent carbon nanoparticles suspension injection subcutaneous injection through the bilateral groin to tracer lymph nodes.Twenty-two patients in control group had no tracer injected before surgery.All patients underwent laparoscopic resection of the bladder combined with standard pelvic lymph node dissection(male patients underwent prostatectomy and bilateral seminal vesical resection at the same period,female patients with simultaneous and bilateral attachment hysterectomy,according to the patient’s physical condition and other factors,somepatients underwent ureteral skin ostomy,some patients underwent ileal neobladder surgery).Observe lymph node black staining of tracer group patients intraoperative,calculate the number of black dye lymph nodes and lymph node black staining rate in tracer group.The number pathologically detected lymph nodes,the number of positive lymph nodes,the time of surgery,the amount of intraoperative blood loss and the incidence of perioperative lymphatic fistula were compared between the two groups to evaluate the safety and effectiveness of lymphatic tracer on bladder cancer radical surgery.Results: In the tracer group,intraoperative lymphadenectomy was effective and there were no tracer-related side effects and complications in the perioperative period.By comparing the number of lymphadenectomy nodes,we found that Canarine can significantly improve the clearance and detection rate of lymphadenectas(P=4.63E-7),and the rates of black staining in lymph nodes were higher than 50% in all patients,even reach to 85%.However,no significant difference was found in the time of surgery and the number of positive lymph nodes between the two groups(P>0.05).There was also no difference between the two groups in the amount of bleeding and other complications(P>0.05).Conclusion: 1 、 In laparoscopic radical cystectomy,the application of carbonnano-lymph node tracer-Canarine in precise pelvic lymph node dissection is safe and without side effects.2 、 In laparoscopic radical cystectomy,the application of carbon nano-lymph node tracer-Canarine has positive effect in lymph node dissection and lymph node pathology detected.
