Objective:to compare ultrasound(US)with full-field digital mammography(FFDM).To evaluate the value of small masses in different breast types..Methods:562 patients with small breast masses confirmed by operation or puncture(maximum diameter<2.0 cm)who had complete US and FFDM data and pathological results.according to the data of FFDM,the selected patients were divided into four types:There were 59 patients with all-fat breast(ACR type a),187 with diffuse fibrous glandular type(ACR type b).There were 195 cases of ACR c and 121 cases of ACR d of dense mammary gland.The detection results of US and FFDM in different types of small breast masses and the BI-RADS grading of small masses were analyzed by double-blind analysis,and then compared with the pathological results.The sensitivity,specificity and accuracy of US and FFDM in the diagnosis of small masses in different types of mammary glands were analyzed,and the consistency of kappa was analyzed.Results:among the 562 patients,279 were benign lesions and 283 were malignant lesions.1.In ACR a class of breast,the detection rate of US lesions was 91.53%,sensitivity was 71.18%,specificity was 72.88%,accuracy was 74.57%.the detection rate of FFDM lesions was100%,sensitivity was 83.05%,specificity was 81.36%,accuracy was 86.44%.The detection rate,sensitivity,specificity and accuracy of MTM for small masses were higher than those of US(P<0.05).US and FFDM assess consistency(Kappa=0.47).2.In ARC b class of breast,the detection rate of US lesions was 97.32%,sensitivity was 77.54%,specificity was 74.33%,accuracy was 75.94%.The detection rate of FFDM lesions was 97.86%,sensitivity was 78.07%,specificity was 75.40%,accuracy was 79.68%.The detection rate,sensitivity,specificity and accuracy of MTM for small masses were no significant difference US(P>0.05).Evaluation consistency is high.(Kappa=0.78).3.In ARC c class of breast,the detection rate of US lesions was 97.94%,sensitivity was 75.09%,specificity was 78.46%,accuracy was 77.44%.The detection rate of FFDM lesions was 96.92%,sensitivity was73.33%,specificity was 76.41%,accuracy was78.97%.The detection rate,sensitivity,specificity and accuracy of MTM for small masses were no significant difference US(P>0.05).Evaluation consistency is high.(Kappa=0.76).4.In ACR d class of breast,the detection rate of US lesions was 96.69%,sensitivity was 78.51%,specificity was76.03%,accuracy was 79.37%.the detection rate of FFDM lesions was 76.85%,sensitivity was 52.89%,specificity was 56.19%,accuracy was 57.85%.The detection rate,sensitivity,specificity and accuracy of US for small masses were higher than those of FFDM(P<0.05).The evaluation of US and FFDM is poor.(Kappa=0.35)2..Conclusion:Molybdenum target radiography is superior to ultrasound in the treatment of small breast masses of ACR a.There was no significant difference between ultrasound and mammography for small masses in ACR b and ACR c mammary glands.Ultrasound was superior to mammography in the case of small masses in ACR d mammary gland.
