Effect Of Cardiopulmonary Rehabilitation Comprehensive Treatment On Quality Of Life In Stable Chronic Obstructive Pulmonary Disease Elderly Patients

PurposeThe study is to promote the health of stable chronic obstructive pulmonary disease elderly patients,use the comprehensive intervention model of cardiopulmonary rehabilitation,and understand the effects of the cardiopulmonary function,quality of life and mood state of these patients,and hope to provide reference for the rehabilitation nursing of clinical nursing staff.Method(1)Choosing Hunan People’s Hospital from January 4,2016 to February 28,2017 in the elderly patients with chronic obstructive pulmonary disease in the Department of respiration,the comprehensive Department of respiration and the dry care department.According to the random grouping method,the new hospitalized patients with chronic obstructive pulmonary disease in the stable period are in accordance with the criteria.Serial numbers were allocated to intervention group(55 cases)and control group(55 cases)in turn.(2)The general information questionnaire,BODE index,St Georges Respiratory Questionnaire,Profile of Mood States as the investigation tool to carry out baseline survey,110 stable patients with chronic obstructive pulmonary disease(patients with complete data files)to investigate,understand stable period of elderly patients with chronic obstructive pulmonary disease patients sense of coherence,quality of life and mood state.(3)Tasked on the beneficial health model,combined with Maslow’s needs theory,the theory of golden standard and the theory of positive psychology,the intervention mode of cardiopulmonary rehabilitation for stable elderly patients with chronic obstructive pulmonary disease was studied.The patients in the control group were given rehabilitation instruction according to the routine treatment,while the intervention group received 3 months’ intervention in the comprehensive intervention mode of cardiopulmonary rehabilitation while the routine rehabilitation instruction was conducted.Then the cardiopulmonary function,BODE index,SGRQ score and TMD score of the two groups were compared.(4)SPSS24.0 statistical software was used for statistical analysis.Statistical methods such as mean,standard deviation and percentage were used for statistical description.χ2 test and t test were used for statistical analysis.Result(1)Before the intervention,the intervention group and the control group of two groups of stable period of elderly patients with chronic obstructive pulmonary disease patients with general demographic data,BODE index(BMI,6MWD,Borg,FEV1%),quality of life score and mental state SGRQ(POMS)TMD total scores by comparison,there were no significant differences(P>0.05).(2)After cardiopulmonary rehabilitation intervention,the intervention group participants in the intervention group before and after the comparison,FEV1%pred and FEV1/FVC were significantly higher than those before intervention(P<0.05);and the results were compared between groups with the control group,the control group intervention group than the lung function index.A statistically significant difference(P<0.05).(4)In the intervention group,the scores of cardiac function indexes such as anaerobic threshold(AT)and carbon dioxide ventilation equivalent slope (VE/VCO2)were significantly higher than those before intervention,and the difference was statistically significant(P<0.05).(4)After cardiopulmonary rehabilitation intervention,subjects in the intervention group compared with before intervention,body mass index,BMI 6MWD,Borg and m MRC scores were better than those before the intervention,significant difference(P<0.05);compared with the control group,the BODE index was significantly better than the control group(P<0.05),namely the cardiopulmonary rehabilitation comprehensive intervention,the intervention group subjects to enhance the nutritional status improved,reduce the degree of airflow obstruction,dyspnea,exercise capacity decreased.(5)After cardiopulmonary rehabilitation intervention,subjects in the intervention group compared with before intervention,the total scores of SGRQ decreased(P<0.05);compared with the control group,SGRQ score was low(P<0.05),showed that after cardiopulmonary rehabilitation intervention,the intervention group subjects improved the quality of life.The intervention group compared with the control group,the three dimensions of St Georges respiratory questionnaire in the intervention group received cardiopulmonary rehabilitation intervention,respiratory symptoms,improve exercise capacity has been enhanced,effects of disease on daily life is reduced,the difference was statistically significant(P<0.05).(6)After the comprehensive intervention of cardiopulmonary rehabilitation,the total score of TMD was significantly lower than that before intervention(P<0.05).Compared with the conventional control group,the TMD score was lower,and the difference was significant(P<0.05).The results showed that after the prognosis of cardiopulmonary rehabilitation,the mood of the subjects in the intervention group was improved.Comparing the two kinds of emotions in the POMS scale between the intervention group and the control group before and after intervention,we found that the intervention group had a negative mood score and a positive mood score after receiving the cardiopulmonary rehabilitation intervention,and the difference was statistically significant(P<0.05).Conclusion(1)The cardiopulmonary rehabilitation comprehensive intervention model based on the beneficial health model can effectively improve the cardiopulmonary function index of elderly patients with stable chronic obstructive pulmonary disease.(2)The comprehensive intervention model of cardiopulmonary rehabilitation plays a positive role in the quality of life of the patients with chronic obstructive pulmonary disease in the stable period.(3)The comprehensive intervention model of cardiopulmonary rehabilitation can reduce the negative emotion of the patients with chronic obstructive pulmonary disease in the stable period and guide the positive emotion.
