The Value Of DNA Ploid Analysis And Thinprep Cytology Test In Cervical Disease Screening:A Meta Analysis

Objective:To compare the screening value of DNA ploid analysis and Thinprep cytology test in cervical disease through the meta analysis.Methods:Searching the Chinese and English databases,as CNKI,Wangfang,CBM,Pubmed,Cochrane library and Embase,by computer from inception to 2018 february with key words,for example “Thinprep cytolog* test”“ploidy” “cervical cancer”and so on,we retrieve all the studies which are relevant to screen CINⅡby thinprep cytology test and DNA ploid analysis,with the selection and delection standard.We extracted the four-fold table data from the selected studied,which contain ture positive data,false positive data,ture negative data;false negative data,assessed methodological quality by the software of Review Manager5.2,pictured deeks funnel and evaluated the publication bias by STATA,counted sensitivity,specificity,DOR,and AUC value by the software of metadisc,a specially software for diagnosis meta analysis.After that,we compared the result between TCT and DNA ploid analysis,got thebetter one on screening cervical disease.Results:There is 14 studies selected with strict selection and delection standard,which are all from China.One of they is retrospective study,others are cross-sectional study.All the study contain 101111 patients,actually biopsy case is5078.the TCT involved is 4524 patients,the number of patient who carried out DNA ploid analysis is 5078.The incoporate result of meta analysis by metadisc shows:the sensitivity of TCT is 0.54(95%CI:0.51-0.57);the specificity is 0.82(95%CI:0.81-0.84);the positive predictive value is 2.73(95%CI:1.69-4.41);the negative predictive value is 0.59(95%CI 0.48 0.72);DOR value is 5.88(95%CI:2.79-12.38),and the AUC value is 0.7151.The incorporate consequenses of DNA ploid analysis are as follow: the sensitivity is 0.82(95%CI:0.80-0.84);the specificity is 0.75(95%CI:0.73-0.76);the positive predictive value is 2.98(95%CI:2.06-4.29);the nagetive predictive value is 0.23(95%CI:0.14-0.38);the DOR value is17.46(95%CI:7.48-40.73);and the AUC value is 0.9005。All the result is statistically significant.Conclusion:The value of screening CINⅡand above lesions of DNA ploid analysis system was obviously better than that of TCT.So,DNA ploid analysis is a reliable and effective way of screening CINⅡand above lesions.
