Objectives:The aim of this study was to compare the bowel preparation quality, the patient tolerability, the cecal insertion rate and the willingness to repeat of different dosage regimens polyethylene glycol electrolyte powder for bowel preparation for morning colonoscopy patients. Methods:Continuous inclusion total 144 patients who came to digestive department of our hospital undergoing morning colonoscopy and randomly assigned into A,B,C groups by the methed of drawing lots.Subjects in group A drank 2L PEG the evening before the colonoscopy;group B receive 2L PEG the morning of the colonoscopy;and the group C receive 1L PEG the evening before the colonoscopy, and another 1L PEG the morning of the colonoscopy for bowel preparation.Compared the evaluation index include bowel cleansing quality, the incidence of adverse reactions(for example:nausea, vomiting, abdominal pain, abdominal distention,et al),sleep disturbances, willingness to repeat and cecal insertion rate.Analysis the effictions of different dosage regimens Polyethylene glycol electrolyte powder for bowel preparation quality and the Patient tolerance for morning colonoscopy patients,in order to put forward a way of bowel preparation which is higher bowel preparation quality and better tolerance. So as to improve the effectiveness and reliability of colonoscopy about morning colonoscopy patients.Results:1.The success rate of bowel cleaning about A,B,C groups(84.3%vs 80.4%vs 89.4%)have no significant differences(p>0.05),but the level I rate of group C was significantly high than group A and B(19.6%vs 21.7%vs 46.8%,p<0.05),that indicated the dosage regimen of group C can obtain more higher quality of bowel cleaning effect.Logistic regression analysis factors about the level I of bowel cleaning effect have revealed that dosage regimens was main factors associated with the level I of bowel cleaning effect(p<0.05).2.The incidence of adverse reactions of C group was lower than group A and B(66.7%vs 65.2%vs 36.2%),the differences between 3 groups were statistically significant(p<0.05),In all adverse reactions, the incidence of bloating about group C was 4.3%,lower than group A and B(p<0.05),but futher comparisons found that the only the incidence of bloating about group A and C were statistically significant(p<0.05),the split dose bowel preparation can significant lower the incidence of bloating compared with single dose bowel preparation regimens in evening before the colonoscopy.Logistic regression analysis factors about the incidence of adverse reactions have revealed that dosage regimens was main factors associated with the incidence of adverse reactions(p<0.05).3.In this study, the sleep disturbances about A,B,C groups were 37.3%,45.7%,19.1%,the differences were statistically significant(p<0.05),Futher comparisons found that the sleep disturbances about group B and C were statistically significant(p<0.05),but there were no significant differences between group A and C,group A and B(p>0.05);The willingness to repeat in C were higher than groups A and group B(96.1%vs 76.1%vs 97.9%,p<0.05),Futher comparisons found that the willingness to repeat about group B and C were statistically significant(p<0.05),but there were no significant differences between group A and B,group A and C(p>0.05).Logistic regression analysis revealed that the incidence of adverse reactions was main factors associated with the sleeping(p<0.05),and the incidence of adverse reactions and sleep disturbances were main factors associated with the willingness to repeat(p<0.05).The willingness to repeat was more higher in male, the compliance was better than female(p<0.05).4.All patients have complete the colonoscopy, the cecal insertion rate of three groups were 100%.Conclusion:1.Split-dose regimens of polyethylene glycol electrolyte powder for morning colonoscopy can gain the same effective and cecal insertion rate as taking the whole-dose on the same morning or on the evening before the colonoscopy, but the level I rate was significantly higher.2.The patients with split-dose regimens had lower incidence of adverse reactions, especially, the incidence of bloating was significantly lower than taking the whole-dose on the evening before the colonoscopy. And split-dose regimens of polyethylene glycol electrolyte powder for bowel preparation can decreased the incidence of sleep disturbances, increased patient willingness to repeat, patients were well tolerance. The dosage regimens was main factors associated with the level I of bowel cleaning effect and the incidence of adverse reactions,so,we can increased the bowel preparation quality and decreased the incidence of adverse reactions by take different dosage regimens, and decreased the incidence of sleep disturbances, increased patient tolerance.
