| Objectives To explore the relationship between serum magnesium level and dyslipidemia in patients with Maintenance hemodialysis(MHD)by analyzing the changes of serum magnesium level in patients with dyslipidemia,and to provide theoretical basis for clinical intervention.Methods From October 2016 to October 2017,94 patients with maintenance hemodialysis were studied at the law of blood purification center in the Affiliated Hospital of North China University of Polytechnic University.According to the diagnosis standard of blood lipid abnormality,it was divided into abnormal blood lipid group and normal blood lipid group,and the abnormal blood lipid group was used as the case group,and the normal blood lipid group was used as the control group.The basic information of the patient’s height,weight,sex,age,history,age of dialysis,drug use and the primary disease of ESRD were recorded.The fasting blood glucose were detected(FBG),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),triglyceride(TG),serum creatinine(Cre),urea nitrogen(BUN),albumin(ALB),serum calcium(Ca),phosphorus(P)and magnesium(Mg),parathyroid hormone(iPTH)and other laboratory indexes,SPSS17.0 software is used for statistical analysis.Measurement data were expressed by((x|-)±s).T test was used for comparison between groups.Chi square test was used for chi square test,and Logistic regression analysis of two categories was used for multivariate analysis.P<0.05 showed significant difference.Results 1 The age,sex and body mass index of the two groups were not statistically significant.The age of hemodialysis patients in dyslipidemia group was higher than that of dyslipidemia group(P<0.001).For primary disease of chronic glomerulonephritis,hypertensive nephropathy,diabetic nephropathy,lupus nephropathy,polycystic kidney and other unexplained nephrotic ratio in the two groups,the difference was not statistically significant,dyslipidemia group renal failure was significantly higher than the normal lipid group(χ~2=6.925,P=0.008),the difference was statistically significant.2 Between the two groups,serum creatinine(Scr),urea nitrogen(BUN),calcium(Ca)had no significant difference.Intact parathyroid hormone(iPTH)and fasting blood glucose(FBG)of dyslipidemia group were higher than that of normal blood lipid group.Albumin(ALB),calcium(Ca)and calcium-phosphorus product(Ca*P),phosphorus(P)of dyslipidemia group were significantly lower than that of the normal lipid group(P<0.045),the difference was statistically significant.3 The level of serum magnesium in abnormal blood lipid group was significantly lower than that of the normal blood lipid group.The serum magnesium level was less than or equal to 0.975 for the number of exposure,the level of blood lipid in abnormal group decreased serum magnesium was significantly more than the normal lipid group,MHD decreased serum magnesium could increase the risk of patients with dyslipidemia,which was 2.724 times higher than in patients with normal blood lipid(OR=2.724,P=0.006).The relationship between serum magnesium level and the abnormality of TG and HDL-C was statistically significant.When the serum magnesium level decreased,the risk of TG and HDL-C abnormalities increased.But the relationship between serum magnesium level and the abnormal TC and LDL-C was not statistically significant.4 The risk of dyslipidemia was increased in MHD patients with lower serum magnesium level,longer dialysis age,renal failure and elevated iPTH level.The relationship between blood magnesium and blood lipid abnormality was next to the transplanted kidney,which is second in many factors.Conclusions 1 The risk of blood lipid abnormality in the MHD patients with the decrease of serum magnesium level was higher.2 Serum magnesium is mainly related to the abnormality of HDL-C and TG,and the risk of decreasing HDL-C and increasing TG in patients with lower serum magnesium levels is higher. |
