Objectives To explore the current status and influencing factors of health literacy of patients with moderate to severe COPD,and to explore the relationship between health literacy and rehabilitation compliance of patients with moderate to severe COPD.To provide a theoretical basis for improving the rehabilitation compliance of patients with moderate to severe COPD by taking targeted interventions from the perspective of improving the health literacy of patients.Methods In October 2016 to October 2017,555 cases of moderate to severe COPD patients who were discharged from the respiratory medicine of the Affiliated Hospital of North China Polytechnic University and the Tangshan City people’s hospital were selected for the study.The family return visits were conducted at 6~8 weeks after discharge,and a questionnaire was conducted.The self-designed general situation questionnaire,COPD health literacy scale,COPD rehabilitation compliance scale were used to analyze the status of health literacy and rehabilitation compliance of patients and their influencing factors.All data were recorded by Excel software and statistical analysis was carried out by SPSS20.0 statistics software.The general data of moderate to severe COPD patients in the stable period were analyzed by descriptive statistical analysis,single factor analysis was analyzed by t test and variance analysis,and multivariate linear regression analysis was used in multi factor analysis.Results 1 The average score of health literacy in stable,moderate to severe COPD patients is(6.38±2.62),and the patients with good health literacy only account for 29.1%.2 The univariate analysis of moderate to severe COPD at stable stage in patients with health literacy shows: residence,education level,occupation,income,disease years,disease severity,cognitive impairment,health self-assessment,self efficacy of chronic disease,chronic disease self-efficacy,peer support,Internet utilization,discharge rehabilitation guidance and receiving community health services,the difference was statistically significant(P<0.05).3 Multivariate logistic regression analysis showed that residence,education level,occupation,income,severity of the disease,cognitive impairment,self efficacy of chronic disease,family function,peer support,Internet utilization,discharge rehabilitation guidance and acceptance of the community health service were the influencing factors of rehabilitation compliance in patients with moderate to severe COPD.4 The relationship between health literacy and rehabilitation compliance in stable,moderate to severe COPD patients was significantly correlated with health literacy and total score of rehabilitation compliance in all stable and moderate COPD patients(P<0.05).5 Multi factor linear regression analysis further showed that the health literacy level of moderate to severe COPD patients in the stable period was an important factor in rehabilitation compliance(P<0.05).In this study,it could explain 21.9% of the factors affecting the compliance of rehabilitation.Conclusions 1 The healthy quality of patients with moderate to severe COPD was at a low level.2 Residence,level,occupation,income,severity of the disease,cognitive impairment,self efficacy of chronic disease,family function,peer support etc.are the factors influencing the health literacy of patients with moderate to severe COPD.3 The level of health literacy is the influencing factor of the rehabilitation compliance of patients with moderate to severe COPD.The health literacy and rehabilitation compliance score of patients with moderate to severe COPD were positively correlated,and the improvement of health literacy level was helpful to promote the stability and compliance of patients with moderate to severe COPD.
