Objective The modified FOUR score, proposed by ourselves,is aimed to compare the predictability of FOUR score and GCS score after mild, moderate and severe traumatic brain injury(TBI).Methods The modified FOUR score was designed according to the factors such as FOUR score,GCS score,CT and TCD examination. This is a prospective observational study by total of 60 patients with TBI admitted to our hospital patients. They are classified as mild, moderate,or severe for three groups, depending on the GCS,each of the group included 20 patients. Both modified FOUR and GCS scores were determined at the first、third、seventh day in hospital and the outcome of patients are determined at the time out of the hospital by GOS score.Analysis the correlation of predictability of the prognosis of patients in the modified FOUR and GCS score. Results The GCS and the modified FOUR scores were respectively 0.889 + 0.044 and 0.911 + 0.030 under ROC curve of the AUC,and the results indicated that:Both of the two score for the prognosis of patients with have certain relevance, the predictive value of the modified FOUR score on admission of patients with TBI is similar with the GCS score, however, the FOUR score can be used in special patients just as severe coma for sub-categorization of patients with lowest GCS and it also can be a complement of GCS score. Conclusions The predictive value of the modified FOUR score has a meaningful value for patients with TBI,especially for special patients and it also can be a complement of GCS score.
