| Objective: To analysis the effect of forceps delivery on maternal and fetal outcomes due to prolonged second stage of labor under the new labor standard. Methods: 2983 pregnant women were spontaneous labor at the First Affiliated Hospital of China Medical University, among them, there were 1701 pregnant women by spontaneous labor from January 2012 to December 2014,and 1788 pregnant women by spontaneous labor from January 2015 to December 2017.A total of 119 women receiving forceps delivery because of prolonged second stage of labor were collected from January2012 to December 2017,G35w+2 to G41w+4,with new labor criteria(study group,n=61)and Friedman labor criteria(control group,n=58).A retrospective study analysis was applied to investigate the relevant confounders of due to prolonged the second stage of labor to use the forceps delivery and the differences between maternal and neonatal outcomes.Result: 1.Under the standard of the old labor process, the percentage of receiving forceps delivery because of prolonged second stage of labor is 3.341%;under the standard of the new labor process, the percentage of it is 3.385%.There is no statistically significant difference between two groups(p>0.05);2.Under the standard of the old and new labor process, receiving forceps delivery because of prolonged second stage of labor, there was no significant difference in postpartum fever, severe perineal laceration(III-IV laceration),maternal urinary retention, neonatal 1 and 5 minutes Apgar score and transferred to NICU(p>0.05).Under the new labor standards,Increased the incidence of postpartum hemorrhage rate of forceps, the result was statistically significant(p<0.05).Conclusion: The implementation of new labor standards, due to prolonged the second stage of labor using forceps delivery,it did not increase the probability of forceps delivery, forceps delivery of maternal and neonatal outcomes without extensive influence, but should pay attention to the prevention of postpartum hemorrhage. |
