Objective:Rheumatoid arthritis(RA)is a common chronic systemic autoimmune disease characterized by arthropathy,and characteristic symptom is symmetry,the peripheral sex multiple joint chronic inflammatory disease,the clinical manifestations of rheumatoid arthritis are pain,swelling and function decline,continuous and recurrent attack,low fatality rate,but high disability rate,up to 40%.Rheumatoid arthritis affects the life and work of patients seriously and causes great psychological pressure and financial burden on patients themselves and their families.The world health organization defines the quality of life as an experience of individuals in different cultures and values that relate to their goals,expectations,standards,and things they care about.In recent years,the quality of life scale had been widely used in the evaluation of life quality of the general population and the affected population,which had effectively improved our understanding of disease and health.Quality of life and its influencing factors research had focused on the south,the north of the RA in the quality of life is blank.Case-control study was applied in this research,it systematically investigated rheumatoid shutdown inflammation in patients with epidemiological data,and then the three kinds of quality of life scale for RA patients and healthy people life quality to evaluate and compare different scale,the analysis of factors influencing the life quality of patients with RA,this study in order to further improve the quality of RA survival and quality of life research.Materials and methods:Case group:There were 200 people from rheumatoid arthritis patients of the first affiliated hospital of China medical university.Control group:There were 200 healthy people from a medical center of military area 202 hospital in Shenyang.And the specific investigation content includes:1)Demographic data;2)Life and environmental factors;3)Behavioral factors;4)disease history,allergy history and family history;5)Clinical biochemical index(just case group);6)Quality of life scales: WHOQOL-BREF,SF-36,QLICD-RA(case and control group).And then data was analyzed by SPSS20.0.In the process of statistical description,the counting data was expressed as a percentage.If the data was in a normal distribution,the mean number±standard deviation could be used to indicate,Instead,used the median and quartile spacing to indicate.And the statistical analysis,the measurement data was analyzed by t or F test,and the counting data was analyzed by chi-square test.In this study,the scores of each dimension of the scale were converted to the standard score.Multivariate linear regression analysis was applied to analyze the risk factor of quality of life.The reliability test of the scale was used as the internal consistency and retest reliability index and the minimum acceptable value was 0.6.In the test of scale validity,exploratory factor analysis was conducted with KMO statistic > 0.70,and P < 0.01 in Bartlett’s spherical test,indicating the conditions for factor analysis.The correlation of the scale was analyzed by Pearson.Two-tailed value of P<0.05 was defined as statistically significant.Results:1.There were statistically significant differences between RA group and healthy control group(P<0.01)for the total score of SF-36,WHOQOL-bref and OLICD-RA scales,in which the SF-36 scale in the overall health,physiological function,physiological function,body pain,vitality,social function,emotional role and mental health in the field of eight score were statistically difference in RA group and healthy controls(P<0.01);There were statistically significant differences between RA group and healthy control group(P<0.01)in the four fields of physiological,psychological,social and environmental fields of WHOQOL-bref scale.There were statistically significant differences between RA group and healthy control group(P < 0.01)in the four fields of physiological,psychological,social and RA of OLICD-RA scale.The quality of life in RA group was lower than that of healthy control group.2.When the SF-36 scale was evaluated,the time of sleep deprivation,psychological counseling,and normal C4 levels entered the final model(P=0.01,P < 0.01,P=0.01).When the WHOQOL-bref scale was used for evaluation,IgA raised and overweight entered the final model(P<0.01,P=0.03);Application to evaluate OLICD-RA scale,getting enough sleep and psychological counseling in the final model(P=0.01,P=0.01),including psychological counseling,normal C4 level,overweight,RA is the protection of the quality of life factors,lack of sleep and increased IgA RA is a risk factor for quality of life.3.SF-36,WHOQOL-bref and OLICD-RA scale scores were highly correlated,among which SF-36 and WHOQOL-bref(r=0.84,P<0.01),SF-36 and OLICD-RA(r=0.92,P<0.01),WHOQOL-bref and OLICD-RA(r=0.87,P<0.01).Conclusion1.The life quality of patients with rheumatoid arthritis in Liaoning was generally lower than that in Liaoning.2.Sleep enough,BMI,psychological counseling,C4 and IgA levels may affect the quality of life of patients with rheumatoid arthritis.3.The results of sf-36,WHOQOL-bref and OLICD-RA scale were consistent.
