Retrospective Analysis Of 372 Clinical Data Of Abdominal Wall Endometriosis After Cesarean Section

Objective: To discuss the methods of diagnosis and treatment of abdominal wall endometriosis(AWE),and provide the best choice for clinical workers.Methods:The data of 372 AWE patients admitted to gynecology ward of Shengjing Hospital from Jan.2012 to Dec.January 2016 were collected,Retrospective analysis of the clinical characteristics and diagnosis and treatment process of AWE patients.Results: A total of 372 patients were analyzed in the present cross-sectional study(mean age 32.4 years).All the patients had cesarean section history.Incubation period significantly prolonged in the patients whose cesarean section age is over 27 years old compared with those under 27(P < 0.05).336(90.3%)patients had Pfannenstiel incision,while 36(9.7%)patients had transcrectal incision.The primary clinical presentations were abdominal pain(93.8%)and masses(98.1%).All the patients received B-scan ultrasonography.There is no difference bet ween the size of the B-scan measurement and the size of the actual focus.(P>0.05)There’s no significant difference in the location of the focus between B-scan,CT and MRI(P<0.05,0.4<Kappa<0.75).Of the 211 patients who assayed serum CA125 level,79.1%had CA125 value in the normal range,and 20.9% had CA125 > 35 U/m L,the mean value is 88.6U/m L.Surgery was performes in all the 372 patients and 427 lesions was resected.330 patients had single lesion,of which mean diameter is 3.14 cm.And 42(11.3%)patients had multiple lesions.16(4.3%)patients required biotic patch for fascial closure in order to prevent ventral hernia.Conclusion: Abdominal wall endometriosis is a hormone dependent disease.The main location of the ectopic endometrium is between the fat layer and the myometrium.B ultrasound is the first choice for diagnosis of AWE.Iatrogenic dissemination plays an important role in the development of AWE.The key to prevent AWE is to strictly control indication of cesarean section and enhance surgical skills.
