Objective: Hypertensive disorder complicating pregnancy is a particular disease during pregnancy,which is mainly characterized by hypertention,proteinuria and disappearance after delivery.At present,the incidence of HDCP in China is between 9.4%-10.4% and is increasing year by year.Epidural block can block the sympathetic efferent fibers segmentally,causing the dilation of the resistance vessels and capacity,and the symptoms of the decrease of blood pressure.The aim of this study was to explore the effect of epidural block for rats with hypertensive disorder complicating pregnancy,and through the observation of blood pressure,weight of placenta and fetal in rats and serum factors to determine whether epidural block has a role in the treatment of hypertensive disorder complicating pregnancy.Method: In this study,healthy female and male Sprague-Dawley rats were selected.They were divided into A group(control group),B group(RUPP group),C group(RUPP operation + normal saline group)and D group(RUPP operation + epidural block group)(n=10).On gestation day(GD)14,rats in control group underwent a sham procedure.The rest three groups were established the hypertensive disorder complicating pregnancy model by RUPP operation on the fourteenth day of gestation,and the epidural catheter was placed on the same day in group C and group D.The pregnant rats in group C were treated with normal saline 25?l,two times a day until cesarean section.Rats in group D was given 0.125% bupivacaine 25?l,two times a day until cesarean section.The blood pressure of the pregnant rats were detected at nineteenth days of pregnancy and all rats do caesarean section on the same day.The weight of fetal in all groups and the weight of the corresponding placenta were measured,and the levels of VEGF and s Flt-1 in all groups of pregnant rats were measured.Results: 1.The blood pressure of pregnant rats in group B,group C and group D was significantly higher than that in group A.(P<0.05)There was no significant difference in blood pressure between group B(147.60 + 6.00 mm Hg)and group C(148.02 + 6.39 mm Hg).Compared with group C(148.02 + 6.39 mm Hg),the blood pressure was significantly lower in group D,but it was still higher than that in group A(80.82 + 4.64 mm Hg).(P<0.05)2.The difference between the four groups of weight of fetal in all groups and the weight of the corresponding placenta was similar to that of the blood pressure,but the placental weight of the pregnant rats in group A(0.44 + 0.02g)was not significantly different from that in the D group(0.45 + 0.01g).(P<0.05)3.The concentration of serum VEGF in group B and group C was significantly lower than that of the A group.(P<0.05)The serum VEGF concentration of the pregnant rats in D group rose slightly,but there was a certain difference between the group D and group A.The concentration of s Flt-1 in the serum of pregnant rats in each group was opposite to the concentration of VEGF.Conclusion: 1.The RUPP method used in this experiment basically simulates the symptoms of hypertensive disorder complicating pregnancy,and has successfully established a rat model of HDCP.2.Through the observation of mean arterial pressure,fetal and placental weight,changes in the levels of VEGF and s Flt-1 in serum of pregnant rats can be found that epidural block has a certain therapeutic effect on rats with hypertensive disorder complicating pregnancy in this experiment and effectively relieve symptoms of fetal growth restriction.
