| Objective:To observe the efficacy and quality of life of patients recieved regular high flux hemodialysis.Methods:134 patients who recieved regular hemodialysis treatment for more than 6months in Blood Purification Center of Gansu Provincial People’s Hospital,were recruited.The patient’s age,gender,primary disease,education,age of dialysis,family average monthly income,health insurance type and other data were collected.Patients were divided into low flux hemodialysis(LFHD)group and high flux hemodialys(HFHD)group based on blood purification methods;modified quantitative subjective global assessment(MQSGA)was used to assess the patient’s nutritional status;the MOS 36 item short from health survey(SF-36)was used to assess the quality of life of the patient.The patient’s hemoglobin,total protein,albumin,blood lipid,intact parathyroid hormone(iPTH),serum calcium,serum phosphorus,C-reactive protein,urea nitrogen,β2-microglobulin and other laboratory indicators were tested.Results:1.There was no statistical difference in basic data such as age,gender,primary disease,marital status,education level,family average monthly income,age of dialysis,URR,Kt/V,BMI in patients with LFHD and HFHD(P>0.05).2.Compared with LFHD group,cholesterol,low density cholesterol,triglyceride,C-reactive protein and iPTH were significantly lower in HFHD group.Hemoglobin,total protein,albumin,high density cholesterol,and clearance rates ofβ2-microglobulin were significantly improved in HFHD group.The difference was statistically significant(P<0.05).There was no significant difference in serum calcium and phosphorus between two groups(P>0.05).3.Compared with LFHD group,the scores of MQSGA were significantly lower in HFHD group(P<0.05).4.The scores of SF-36,physical component summary(PCS)and mental component summary(MCS)in males were higher than those in females,and the difference was statistically significant(P<0.05).5.With the boundary of 60 years old,the SF-36,PCS,and MCS scores of middle-aged and young patients were higher than those of elderly patients,and the difference was statistically significant(P<0.05).6.Compared with LFHD group,the scores of the SF-36,PCS,physical function(PF),role physical(RP),body pain(BP)and general health(GH)were significantly higher in HFHD patients(P<0.05).In the MCS,validity(VT),social function(SF),role emotional(RE),and mental health(MH),there was no statistical difference in scores between groups(P>0.05).7.The SF-36 scores of young patients in the HFHD group were significantly higher than those in the LFHD group(P<0.05).The PCS scores of the young and elderly patients in the HFHD group were significantly higher than those in the LFHD group(P<0.05).8.Compared with the same sex SF-36 and PCS between the two groups,the score of female patients in HFHD group was significantly higher than that of LFHD group,and the difference between the two groups was statistically significant(P<0.05).9.Person linear correlation results showed that age,hemopurification modes,MQSGA,hemoglobin,total protein,and iPTH were related to SF-36(P<0.05).Age,hemopurification modes,MQSGA,hemoglobin,total protein,albumin,β2-microglobulin clearance,iPTH and triglyceride were associated with PCS(P<0.05).There was a correlation between age and sex with MCS(P<0.05).10.Multiple stepwise linear regression analysis showed that age was negatively correlated with SF-36,PCS and MCS,and age and hemopurification modes were independent factors of SF-36 and PCS(P<0.05).Age and sex were independent factors of MCS(P<0.05).Conclusions:1.High-flux hemodialysis can effectively improve the patient’s dialysis efficacy and nutritional status,can improve the physical health of patients of all ages and female patients,then improve the quality of life of patients.2.Blood purification is an independent influencing factor of quality of life and physiological health of MHD patients.Age and gender are independent influencing factors of mental health of MHD patients. |
