The Hughston grade III injury of medial collateral ligament(MCL)often combines with the anterior cruciate ligament(ACL)injury. In the clinic, ligament reconstruction is the major treatment till the present moment. The treatment of ACL injury has evolved from isometric reconstruction to anatomical reconstruction which can bring better stability and activity levels. Most of the treatment of MCL injury cannot recover rotational stability.Importantly, from a functional perspective, the MCL and ACL both contribute to maintaining rotational stability and the rotatory instability would be aggravated if these 2 structures were simultaneously injured. We present a clinical treatment strategy for the combined injury of MCL and ACL,which aims as a better guidance and application in the clinic.
