| Part one Correlative Study of Coronary Vein System of Normal Adult in Different Phase of Cardiac Cycle Using 256-slice Spiral CTObjective: To quantitatively measure of normal adult coronary system parameter in different phase of cardiac cycle by 256-slice computed tomography, and to provide normal reference ranges of coronary sinus, mainly branch vein diameter, sectional area in different phase of cardiac cycle. Methods: A total of 70 patients(30 men,40 women;mean age49.89±10.85 years)with suspected coronary heart disease examined by MSCT were selected and patients with physical examination were also selected. The result of MSCT showed no coronary artery disease(no coronary artery deformity, variation)and all patients are right dominant type of coronary artery.Systole phase(45%)and diastolic phase image(75%)of the original image were reconstructed, and quantitative measured coronary sinus, mainly branch diameter, sectional area, and to analyse the relationship between the above imaging indexes with the different phase of cardiac cycle.Results:1.During the different phase of cardiac cycle, the data measured of coronary sinus, mainly branch diameter, sectional area are larger in 45% phase than those in 75% phase(P<0.05).2.During the same phase of cardiac cycle, the coronary sinus, mainly branch diameter, sectional area are no significantly difference between males and females(P>0.05),there were similar between males and females in the above imaging indexes according standardization heights and weights(P >0.05).3.During the 45% interval and 75% interval, there showed significantdifference in the 95% reference ranges of coronary sinus, mainly branch diameter, sectional area. Conclusion :The coronary venous system parameters can be measured by 256-slice spiral CT.The diameter and sectional area of the coronary sinus and its branch veins vary with different phase in cardiac cycle. The reference value of the normal coronary vein in different phases is suggested.Clinical cardiac interventions provide richer imaging information.Part two Correlative Study of the Patients with Triple Vessel Lesion Coronary Artery Disease in Different Phase of Cardiac CycleObjective:To quantitatively measure of the patients with triple vessel lesion coronary artery disease coronary system parameter in different phase of cardiac cycle by 256-slice computed tomography, and to analyse the relationship between the above group with control group. Methods: A total of 100 patients with suspected coronary heart disease examined by MSCT were selected. There have 50 patients showed triple vessel lesion coronary artery serious disease(right coronary artery, left anterior descending branch, left circumflex artery). It named the triple vessel lesion coronary artery disease group.Another 50 patients showed no coronary artery disease. It named control group.Systole phase(45%)and diastolic phase image(75%)of the original image were reconstructed, and quantitative measured coronary sinus, mainly branch diameter, sectional area, and to analyse the relationship between the above imaging indexes with the different phase of cardiac cycle. Results :1.There are no significantly difference between triple vessel lesion coronary artery disease group and control group in age and gender(P>0.05).2.During the different phase of cardiac cycle, the data measured of coronary system parameter are larger in 45% phase than those in 75% phase(P<0.05).3.During the 75% phase of cardiac cycle, the coronary sinus, mainlybranch diameter and sectional area are no significantly difference between triple vessel lesion coronary artery disease group and control group(P>0.05).During the 45% phase of cardiac cycle, there showed significant difference between triple vessel lesion coronary artery disease group and control group in the coronary sinus in anteroposterior diameter, diameter and sectional area, the left marginal vein diameter and sectional area. However, there were similar between triple vessel lesion coronary artery disease group and control group in Coronary sinus vertical diameter, left ventricular posterior veins, posterior interventricular vein diameter and sectional area. Conclusion: 256-slice spiral CT can be used to measure the coronary vein main branch diameter, coronary vein sectional area. The 45% phase of cardiac cycle can objectively reflect the effects of triple vessel lesion coronary artery disease on the parameters of the coronary vein, which is helpful to explore the influence mechanism in cardiac function of coronary heart disease and provide important imaging information for clinical surgery.Part three Correlative Study of Coronary Vein System in the Patients with Atrial FibrillationObjective: To quantitatively measure coronary system parameter in the patients with atrial Fibrillation by 256-slice computed tomography, and to analyse the relationship between the above group with control group. Methods: 256-slice computed tomography was conducted to image coronary artery and left atrium in two groups.Atrial fibrillation group,50 patients with AF and all patients decided to have an operation of radiofrequency ablation.Control group,54 patients with no coronary artery disease examined by MSCT.Diastolic phase image(75%)of the original image was reconstructed, and quantitative measured coronary sinus, mainly branch diameter, sectional area, and to analyse the relationship of the above imaging indexes between AF group with control group.Results:1.There are no significantly difference between atrial fibrillation groupand control group in age and gender(P>0.05).2.During the 75% phase of cardiac cycle, the coronary sinus, mainly branch diameter and sectional area are significantly difference between atrial fibrillation group and control group(P<0.05).3.However,there is no significant difference between atrial fibrillation group and control group in the posterior interventricular vein diameter and sectional area. Conclusion :256-slice spiral CT can be an objective quantitative measurement the parameters of coronary venous system in patients with atrial fibrillation.Atrial fibrillation has a significant impact on the CT parameters of the coronary vein. That lays an important anatomical basis for clinical radiofrequency ablation. |
