| With the development of modern medical treatment,spinal fusion surgery has been widely used in clinic.But the incidence of lesions in the adjacent stage after spinal fusion also increased,so that the patient can again appear low back pain,nerve damage manifestations of clinical symptoms.Due to the increasing research on the biomechanical and physiological functions of the spine,it was found that the motor function of fusion stage disappeared after spinal fusion,which caused the change of intervertebral stress conduction.The pressure of intervertebral disc and facet joint also increased significantly in adjacent stage,which resulted in degeneration of adjacent stage after fusion.Through the retrospective analysis of this literature.It was found that the follow-up time,choice of operation mode,sex,age,lesion degree of adjacent stage and osteoporosis were closely related to the adjacent stage of degeneration and spinal fusion.The pathogenesis is the biomechanical changes in the adjacent stage after lumbar internal fixation,the changes of stress and mobility of the lamina and posterior joints;After lumbar fusion,the motion of fusion segment disappeared,and the range of adjacent segments increased compensatory,which resulted in pathological changes of adjacent segments.Therefore,by studying the risk factors and pathogenesis of adjacent stage degeneration,we can help clinicians to reduce the risk before,after and during operation,thus reducing the incidence of postoperative ASD.Improve the quality of life of patients. |
