Objective:To analyze the relationship between plasma homocysteine,folate and vitamin B12 levels and the related genes polymorphism and adverse pregnancy in folate metabolism,so as to provide objective,reliable scientific basis and medical guidance for primary prevention of birth defects,so as to prevent the recurrence of bad pregnancyResearch object:Collect 2000 samples of pre-pregnancy counseling women and their spouses in the outpatient clinic of the First People's Hospital of Yunnan Province from July 2017 to December 2017 and to fill in the folic acid supplement questionnaire.The subjects were divided into two groups:the adverse pregnancy group and the normal pregnancy group based on the statistically valid questionnaires.The history of adverse pregnancy includes:Down syndrome?DS?,premature birth,spontaneous miscarriage,children with abnormal brain development,other chromosomal aberrations,premature delivery of fetal malformation,restricted fetal growth,or cessation of development;the levels of plasma homocysteine,RBC folate,serum folate and vitamin B12 were measured in each group.The adverse pregnancy was divided into taken folate during pregnancy group and not taken folate during pregnancy group according to the supplementation of folate,and to compare the content of plasma homocysteine-the level of reference range is 5-15?mol/L,RBC folate-the level of reference range is 1328.7-4315.2 nmol/L,serum folate-the level is13.4-56.2 nmol/L and vitamin B12-the level is 180-914 pg/mL.Analyze the genetic frequency of the folate metabolism related gene loci MTHFR C677T,MTHFR A1298C,MTRR A66G,MTR A2756G,RFC1 A80G,MTHFD G1958A based on the400 cases of adverse pregnancy and normal pregnancy by statistically valid questionnaire.The correlation analysis of plasma homocysteine and erythrocyte folic acid and folate polymorphism was performed on the 800 samples.Methods:Experiment was performed by BECKMAN COULTER UniCel DxI 800 Access Immunoassay system,Dia Sorin LIAISON automated immunoassay analyzer,ACQUITY Ultra Performance Liquid Chromatography Tandem Mass Spectrometry System and Polymerase chain reaction and gene sequencing technology to measured respectively the levels of serum folate and vitamin B12,RNC folate,homocysteine,and folate metabolism related gene polymorphis.Statistical Analysis:Excel was used to record data,Statistical data was using SPSS,and taking measures of?2 test and analysis of variance to analyze.P value of<0.05 was considered to indicate statistical outstanding significant.Results:The statistically valid questionnaire data shows that 636 females with age?28.7±5.1?years were in the final adverse pregnancy group,and 435 women in the normal pregnancy group with age?29.5±4.3?years.The levels of homocysteine and serum folate in the normal pregnancy group were lower than those in the adverse pregnancy group,but the levels of RBCl folate and vitamin B12 were higher than those in the adverse pregnancy group.The levels of homocysteine,RBC folate,and vitamin B12 in Patients with normal levels had a better distribution in the normal pregnancy group than those in the unhealthy pregnancy group?P<0.05,significant difference?.The level of serum folate in Patients with normal level in the normal pregnancy group was superior to the normal pregnancy group P>0.05,and there was no significant difference.The level of homocysteine in the group of adverse pregnancy?folic acid during pregnancy?-431 cases with age?27.8±5.2?,was lower than that of the group with adverse pregnancy?folic acid was not taken during pregnancy?-205 cases with age?28.4±5.4?,while the levels of FA,Vit B12,and serum folate were higher than those of women who did not take folate during pregnancy,P<0.05,there are significant differences.The major site affecting adverse pregnancy was the mutation of folate metabolism-related genes MTHFR C677T and MTR A2756G.The adverse pregnancy caused by polymorphisms of MTHFR C677T and MTR A2756G was mainly manifested in the advanced age group.The most significant points affecting homocysteine content are MTHFR C677T and MTR A2756G,and MTHFD G1958A;MTR A2756G and MTHFD G1958A were the most significant influencing factors in erythrocyte folate.Conclusions:The important influencing factors for poor pregnancy were elevated levels of homocysteine,and decreased folate and vitamin B12.The effect of serum folate on adverse pregnancy was not significantly affected in this study.The occurrence of adverse pregnancy were closely associated with homozygous mutations of the polymorphic loci MTHFR C677T and MTR A2756G.The occurrence of adverse pregnancy increases with maternal age.The most significant points affecting homocysteine content are MTHFR C677T and MTR A2756G,and MTHFD G1958A;MTR A2756G and MTHFD G1958A were the most significant influencing factors in erythrocyte folate.
