Objective:To investigate the subjective and objective sleep quality of patients in COPD group C and D,and to explore the correlation between sleep monitoring parameters and the disease condition,and to provide a reference for the quality of life.Methods:According to the 2017 COPD Global Initiative Guidelines,patients in the C and D groups who were randomly selected to meet the inclusion and exclusion criteria for admission to our hospital were the observation group.The control group was selected as the healthy group who attended the physical examination center of the hospital during the same period.The patient’s general condition was recorded.All patients were performed PSG monitoring except for patients with AHI≥5/h,further improvement of the PSQI and ESS scales,and statistical analysis of the patient’s clinical data.Results:(1)There was 90 patients Of the 63 COPD patients and 30 healthy people in the initial group met the criteria of AHI<5/h.There was no significant difference in age,gender composition ratio,body mass index and neck circumference between the three groups.(P<0.05).(2)Comparison of subjective sleep quality in three groups found that:COPD patients with poor sleep quality(PSQI total score>7 points)a total of 40 cases,the positive rate was 66.68%,9 cases of normal control group,the positive rate was 30.00%;The normal control group had the lowest PSQI score and the highest score in group D(P<0.05).There was no significant difference in ESS scores between the three groups(P>0.05).(3)Comparison of objective sleep quality in three groups found that:Sleep time,sleep efficiency,N2 and N3 were the lowest in the three groups,and sleep latency,awakening time,awakening frequency and N1were the highest,normal control group in contrast,the difference between the groups was statistically significant(P<0.05).There was no significant difference in the REM period between the three groups(P>0.05).(4)The CAT score,mMRC classification,and the number of acute exacerbations in the past 1 year in the D group were higher than those in the C group.The difference was statistically significant(P<0.05).(5)The total score of PSQI was negatively correlated with the proportion of sleep time,sleep efficiency,proportion of stage N2,proportion of stage N3,minimum oxygen saturation at night,and percentage of predicted FEV1,and was positively correlated with the proportion of awakening time,proportion of stage N1,the mMRC grading.(6)The number of acute exacerbations in the past year was positively correlated with sleep latency,proportion of stage N1,CAT score,mMRC classification,and negatively correlated with the percentage of predicted FEV1.Conclusions:There was a decrease in sleep quality of patients with COPD C and D group,and it was more obvious in group D than in group C.The group D had shorter sleep duration,prolonged sleep latency,increased light sleep,decreased deep sleep,and awakened easily.The condition and quality of sleep may be mutually causal,affecting the course of the disease.
