Association Of VKORC1 And CYP2C9*3 Gene Polymorphisms With Warfarin In Patients With Atrial Fibrillation

Object:To investigate the distribution characteristics of CYP2C9*3(1075A>C)and VKORC1(1639G>A)polymorphisms in patients with non-valvular atrial fibrillation(AF),and to compare the Han,Uygur(Uyghur),and Kazak(Kazakh)The patient’s dose of warfarin was used to guide clinical medication.Methods:A total of 241 patients with non-valvular atrial fibrillation who had undergone warfarin genetic testing during hospitalization in the Department of Cardiology,People’s Hospital of Xinjiang Uygur Autonomous Region were selected consecutively.Five patients with incomplete information and two patients with valvular atrial fibrillation were excluded.Microsatellite gene analysis was used to detect the CYP2C9*3(1075A>C)and VKORC1(1639G>A)gene polymorphisms in 234 patients,and to compare the application dose of warfarin in different ethnic groups and the patients of the same ethnic group with different genotypes.I’m applying dose situations.Results: In this study,CYP2C9 was found to have A and C alleles in 234 patients.The frequency of Han allele was 95.1%,4.9%,Uygur frequency was 83.3% and 16.7%,and Kazak frequency was 82.1%.17.9%.The frequencies of AA,AC,and CC genotypes of Han,Uygur,and Kazak nationalities were 91.5%,7.3%,and 1.2% among Han people,76.2%,14.3%,and 9.5% in Uygur,and 78.6%,7.1%,and 14.3 in Kazakh.In this experiment,the CYP2C9*3(1075A>C)genotype and allele were found to differ between Han,Uygur and Kazak.Among 234 patients,VKORC1(1639G>A)had two alleles of G and A.Han allele frequencies were 7.3% and 92.7%,respectively,and Uighur allele frequencies were 38.1% and 61.9%,respectively.Kazakh The frequency of alleles was 21.4% and 78.6%,respectively.The frequencies of GG,GA,and AA genotypes of the Han,Uygur,and Kazak are different,ranging from 1.2%,12.2%,and 86.6% for the Han nationality to 9.5%,57.1%,and 33.3% for the Uyghur,and 7.1%,32.1%,and 60.7 for the Kazakh.%.Chi-squared analysis showed that there were differences in CYP2C9*3 and VKORC1 alleles and genotypes between Han and Uygur,Han and Kazak,but there was no difference between Uygur and Kazak.The Han,Uygur and Kazak ethnic groups have the same genotype warfarin doses.The warfarin doses(mg/day)required for patients with different genotypes of CYP2C9*3(1075A>C)in the same ethnic group were AA type(AC+CC)type(ie Han3.28±0.89> 2.14±0.36;Uygur 3.81±0.97 >2.4±0.52;Kazak 3.45±1.18>2.67±0.52).The warfarin dose(mg/day)required for patients with different genotypes of VKORC1(1639G>A)was(GG+GA)type>AA type(ie,Han 4.18±1.37>3.03±0.71 mg/day;Uygur 3.93± 0.9> 2.57 ± 0.76;Kazak 3.91 ± 1.3> 2.88 ± 0.78).Conclusion: 1.There are differences in the polymorphism distribution of Han and Uygur,Han and Kazak genes.There is no difference in the distribution of gene polymorphisms between Uygur and Kazak ethnic groups.2.Different doses of warfarin in Han,Uygur and Kazak patients have the same genotype.3.The dosage of warfarin was reduced after CYP2C9*3(1075A>C)and VKORC1(1639G>A)gene mutations.
