Objective:To investigate the status of cervical high risk human papillomavirus(HR-HPV)persistent infection in the gynecologic outpatients in Zunyi city, and to for their discuss explore the risk factors of cervical HR-HPV persistent infection,in order to provide some theoretical basis for the etiological prevention and behavioral intervention of cervical diseases in this area. Methods:The clinical data were collected from 1608 cases admitted to three gynecologic outpatients from September 2016 to December 2017,including the Affiliated Hospital of Zunyi Medical College, the First People’s Hospital of Zunyi City and the Maternal and Child Health Care Hospital of Zunyi City. All subjects voluntarily accepted the cervical HPV test.391 cases of HPV positive infection were identified in the first HPV test,of which 227 cases were the HR-HPV infection.40cases were excluded from the 227 infections due to uncompleted questionnaires or unfinished second HPV tests. At last,a total of 187 cases of HR-HPV infection were studied.According to the results of the second HPV tests, the 187 cases were divided into group A(HR-HPV persistent infection group,57 cases)and group B(HR-HPV transient infection group,130 cases). The risk factors of cervical HR-HPV persistent infection in group A and group B were analyzed.Results:1.Among the 1608 cases, the infection rate of cervical HR-HPV was14.12%(227/1608),and the persistent infection rate of HR-HPV was25.11%(57/227).2.Among the 57 cases of HR-HPV persistent infection,45 cases(78.95%)were the single HR-HPV and 12 cases(21.05%)were the mixed HR-HPV(χ~2=18.877,P<0.05).3.The main types of persistent infection of single HR-HPV were type 16(13/45),type 58(10/45),type 52(8/45)and type 51(6/45)(χ~2=47.64,P<0.05).The mixed type was mainly type 58 combined with other types(χ~2=15,P<0.05).4.The number of sexual partners, the times of induced abortion, the use of condom contraception and vaginitis were all associated with the persistent infection of cervical HR-HPV(P all<0.05).Among these related factors, more than 3 sexual partners, more than 3 times abortion, and vaginitis were risk factors of HR-HPV persistent infection(βall>0,OR all>1),while the use of condoms was a protective factor(β<0,OR<1).Conclusions:1.The persistent infection rate of cervical HR-HPV in the gynecologic outpatients in Zunyi City is 25.11%.The single HR-HPV infection play an important role and the main types of infection are type 16,type 58,type 52,and type 51.The mixed HR-HPV infection is dominated by type 58 combined with other types.2.Lowering the number of sexual partners and the times of induced abortion, controlling vaginitis and promoting condom contraception may contribute to reducing the persistent infection of cervical HR-HPV in the gynecologic outpatients in Zunyi City.
