Objective By measuring fetal left ventricular Doppler Tei index and pulmonary venous flow spectrum parameters,to explore its value in the evaluation of fetal left ventricular function in hypertensive disorder complicating pregnancy,in order to provide a basis for clinical monitoring of fetal hypertension during pregnancy.Methods Selecting 130 cases of pregnant women with single or middle pregnancy gestational hypertensive disease in the functional department of Gansu Province Maternal and Child Health Hospital from October 2016 to October 2017 as the experimental group.The experimental group was divided into two groups according to the blood pressure,urine protein and symptoms of pregnant women,mild preeclampsia(MPE)were 84 cases and severe preeclampsia were 46 cases.Another randomly selected 127 normal pregnant women during the same period in the functional department of the Maternal and Child Health Hospital of Gansu Province as the control group.There was no significant difference in the age and fetal gestational age between the experimental group and the control group.Tissue Doppler technique was used to obtain the motion spectra of the experimental group and the control group at the junction of the mitral valve and the left ventricular free wall.The time parameters were measured and collected,and the Tei index was calculated.At the same time,used color Doppler technique and spectral Doppler technique to obtain the pulmonary venous flow spectral parameters in the pulmonary veins into the left atrium,including Ventricular systolic peak velocity of pulmonary vein(S),pulmonary venous velocity during atria systole(A),Pulsation index(PI),resistance index(RI)and S/A ratio.Results1.There was no significant difference in the Tei index between the mild preeclampsia group and the control group(P>0.05);the difference in Tei index between the preeclampsia severe group and the mild preeclampsia group and the control group was statistically significant.Significance(P<0.05);2.There was no significant difference in the ET between the mild preeclampsia group and the control group(P>0.05);The ET difference between the severe preeclampsia group and the mild preeclampsia group and the control group was statistically significant(P<0.05);3.The differences of S-wave,A-wave,PI and RI in pulmonary vein between pre-eclampsia mild group and control group were statistically significant(P<0.05).There was no significant difference in S/A ratio between pre-eclamptic mild group and control group(P>0.05)4.The differences of A wave,PI value,RI value and S/A ratio between the severe group and the control group were statistically significant(P<0.05);the difference of S wave was not statistically significant(P>0.05)5.The difference of S wave,A wave,PI value,RI value and S/A ratio between mild preeclampsia and severe preeclampsia was statistically significant(P<0.05);6.There was no correlation between fetal Tei index and pulmonary vein S wave in pregnancy-induced hypertension(r=-0.189);there was a negative correlation with A wave(r=-0.506);there was a positive correlation with PI(r=0.651)There is a positive correlation with RI(r=0.674);Conclusion1.Tissue Doppler Tei index can reflect the left ventricular overall function of fetuses with hypertensive disorder complicating pregnancy and can provide a basis for clinical monitoring of fetuses with hypertensive disorder complicating pregnancy.2.Pulmonary venous blood flow spectrum parameters can be sensitive to understand the changes of fetal left ventricular diastolic function in hypertensive disorder complicating pregnancy,and have important guiding value for timely clinical treatment.3.Pulmonary Vein Blood Flow Parameters Compared with Tissue Doppler Tei Index Can Be Sensitive to Changes in Left Ventricular Function in Fetal Hypertensive Disorders,it also shows that the first performance of fetal heart function is impaired impaired diastolic function; 4.The combined detection of tissue Doppler Tei index and pulmonary venous blood flow parameters can be more early and more comprehensive response to changes in left heart function of fetuses with hypertensive disorder complicating pregnancy.5.There was no correlation between fetal Tei index and pulmonary vein S wave in pregnancy-induced hypertension.There was a negative correlation with A wave and positive correlation with PI value and RI value.
