Objective: To compare the risk factors and characteristics of carotid ultrasound between acute ischemic stroke older and younger than 80 years old. Methods: From January 2017 to July 2017,52 octogenarians and 300 younger patients(over 64 years old)with acute ischemic stroke, ultrasonographic examination was acquired in those patients. To collect the baseline data and carotid artery ultrasound results, including intima-media thickness, plaque and stenosis, all data was analyzed by SPSS 21.0.Results: 352 patients were enrolled, the very elderly patients, median age was 82.98 years, and the younger patients with a median age of 70.41 years. The prevalence of heart disease, uric acid level and NIHSS in the elderly group were higher than the younger group, while smoking, diastolic blood pressure on admission,ALT,triglyceride, total cholesterol level and LDL level were lower than the younger group.98.1% plaques can be found in the very old,and the younger group was 94.3%.There was no significant difference between the two groups in the detection rate of carotid plaques, the number of plaques and the intima-media thickness. The detection rate of plaques in right common carotid artery and right internal carotid artery is higher than that of the younger group. Most plaques can be found in the carotid artery bifurcation.Unstable plaques were less than the younger group. The degree of vascular stenosis was mostly mild. Conclusion: The older group had more heart disease, less smoking, higher uric acid level, lower total cholesterol level, lower TG level, lower LDL level, lower ALT level, lower diastolic blood pressure on admission.NIHSS scores is higher, cardiogenic cerebral embolism is higher. The plaques of the very elderly were more common in the right common carotid artery and right internal carotid artery. The most common plaques were distributed incarotid artery bifurcation.Unstable plaques were less than the younger group.
